SIDE ENTRY DOOR IS NOT RODENT PROOF. OBSERVED A 1/4' GAP ALONG THE BOTTOM OF THE SIDE ENTRY DOOR TO DIVISION STREET. MANAGEMENT INSTRUCTED TO MAINTAIN ALL OUTER OPENINGS RODENT PROOF. ALSO MUST PROVIDE A CURRENT PEST CONTROL RECEIPT. LAST RECEIPT WAS ISSUED BY TERMINIX ON 11/12/09. SERIOUS VIOLATION 7-38-020.
