All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. FOUND EVIDENCE OF INSECTS ONSITE. FOUND APPROXIMATELY 10-12 LIVE SMALL FLIES LOCATED BEHIND BAR AREAS,BASEMENT DISH WASHING AND PREP ARES AND HOT WATER HEATER ROOM. MUST REMOVE INSECTS,SANITIZE AFFECTED AREAS AND HAVE PEST CONTROL COMPANY SERVICE AREAS. SERIOUS CITATION ISSUED 7-38-020.
The area outside of the establishment used for the storage of garbage shall be clean at all times and shall not constitute a nuisance. OUTSIDE GARBAGE RECEPTACLE NOT INSECT AND RODENT PROOF. FOUND LARGE OPENING IN THE LID CAUSING IT NOT TO BE INSECT AND RODENT PROOF. MUST REPLACE LID. SERIOUS CITATION ISSUED 7-38-020.
All food and non-food contact surfaces of equipment and all food storage utensils shall be thoroughly cleaned and sanitized daily. MUST DETAIL CLEAN ALL FRYERS INCLUDING WHEELS. MUST DETAIL CLEAN ALL TABLES AND BAR SURFACES IN PRIVATE PARTY ROOM AND REMOVE SMOKE RESIDUE.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. MUST WASH,RINSE AND SANITIZE ALL MULTI USE ITEMS IN PRIVATE PARTY ROOM AND REMOVE SMOKE RESIDUE.
The walls and ceilings shall be in good repair and easily cleaned. MUST REPAIR WHOLE IN WALL IN PRIVATE PARTY ROOM AND ALSO REPAIR SMOKE DAMAGED CEILING.
All parts of the food establishment and all parts of the property used in connection with the operation of the establishment shall be kept neat and clean and should not produce any offensive odors. MUST PROVIDE MORE STORAGE RACKS IN BASEMENT AND STORE ALL ITEMS 6 INCHES OFF FLOOR TO PREVENT INSECT HARBORAGE.
The flow of air discharged from kitchen fans shall always be through a duct to a point above the roofline. MUST INSTALL DIPPER WELL NEAR PACKED ICE CREAM FREEZER.
Between uses and during storage ice dispensing utensils and ice receptacles shall be stored in a way that protects them from contamination. PROVIDE THE PROPER ICE SCOOP HOLDERS WHERE NEEDED IN BAR AREAS.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST SEAL SMALL OPENING BETWEEN TWO REAR DOORS LEADING TO THE OUTSIDE GARBAGE AREA.

MUST CLEAN AND MAINTAIN ALL UNCLEAN EQUIPMENT.
The floors shall be constructed per code, be smooth and easily cleaned, and be kept clean and in good repair. OBSERVED ACCUMULATED FOOD/DIRT BUILD UP INSIDE 1ST FLOOR PREP AREA FLOOR DRAINS. MUST CLEAN AND MAINTAIN INTERIOR OF ALL UNCLEAN FLOOR DRAINS.
Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. 2ND FLOOR BAR HANDWASH BOWL AND 3-COMPARTMENT SINK DRAINING SLOWLY. MUST REPAIR AND MAINTAIN SINKS.
All food establishments that display, prepare, or store potentially hazardous foods shall have calibrated metal stem thermometers, provided and conspicuous, for refrigerated and hot food units. MUST PROVIDE THERMOMETERS FOR ALL COOLERS IN PREP AREAS.
