All necessary control measures shall be used to effectively minimize or eliminate the presence of rodents, roaches, and other vermin and insects on the premises of all food establishments, in food-transporting vehicles, and in vending machines. OUTER OPENINGS ARE NOT RODENT PROOF. OBSERVED A 1/2' GAP BETWEEN BOTH SETS OF DOORS TO THE OUTDOOR PATIO AND A 1/4' GAP ALONG THE BOTTOM OF THE SOUTH SIDE ALLEY DOORS. MANAGEMENT INSTRUCTED TO RODENT PROOF ALL OUTER OPENINGS. APPROXIMATELY 10 LIVE FRUIT FLIES WERE OBSERVED IN THE DISHWASH AREA. 5 LIVE COCKROACHES WERE OBSERVED BEHIND THE 2ND FLOOR BAR AREA. APPROXIMATELY 50 MOUSE DROPPINGS WERE OBSERVED IN THE LIQUOR STORAGE ROOM. AFFECTED AREAS MUST BE CLEANED AND SANITIZED.
