FOUND EVIDENCE OF RODENTS ON SITE. FOUND APPROXIMATELY 50 MICE DROPPINGS SCATTERED THROUGHOUT PREMISES IN 1ST FLOOR SUSHI BAR ON SHELVES,IN 1ST FLOOR MAIN PREP AREA ALONG SIDE SMALL CONDIMENT COOLER,BEHIND COOLERS AND FREEZERS IN DISHWASHING AREA,IN BASEMENT UNDER ALL STORAGE SHELVES,UNDER SYRUP DISPENCER AND AROUND HOT WATER HEATER. ALSO FOUND DROPPINGS UNDER SYRUP DISPENCER IN 2ND FLOOR STORGAE CLOSET. MUST REMOVE DROPPINGS,SANITIZE AFFECTED AREAS AND HAVE PEST CONTROL COMPANIES SERVICE THE AREAS. SERIOUS CITATION ISSUED 7-38-020.
All food and non-food contact equipment and utensils shall be smooth, easily cleanable, and durable, and shall be in good repair. MUST SEAL SMALL OPENING ALONG THE BOTTOM OF REAR SIDE DOOR LEADING TO OUTSIDE VALET AREA. REPALCE BROKEN SLIDING GLASS DOOR AT 1ST FLOOR SUSHI BAR DISPLAY COOLER.

The walls and ceilings shall be in good repair and easily cleaned. WALLS BEHIND COOLERS AND FREEZERS IN DISHWASHING AREA ARE IN POOR REPAIR. MUST CLOSE ALL OPENINGS AND HOLES TO PREVENT RODENT ENTRY AND HARBORAGE. ALSO REPALCE ALL CRACKED WALL TILES ALONG NORTH WALL IN 1ST FLOOR PREP AREA. REPAIR CEILING OVER ICE MACHINE IN BASEMENT.
MUST REPAIR Ventilation: All plumbing fixtures, such as toilets, sinks, washbasins, etc., must be adequately trapped, vented, and re-vented and properly connected to the sewer in accordance with the plumbing chapter of the Municipal Code of Chicago and the Rules and Regulations of the Board of Health. MUST REPAIR LEAKY PIPE UNDER DISHMACHINE.
Containers and utensils shall be inverted, covered, or otherwise protected from contamination until used. All single-service drinking straws and containers shall be discarded immediately after use. INVERT ALL MULTI USE ITEMS TO PREVENT CONTAMINATION.

OBSERVED SUSHI PREP COOLER BELOW THE COUNTER AT 47F. UNIT WAS USED TO STORE POTENTIALLY HAZARDOUS FOODS. UNIT TAGGED HELD FOR INSPECTION. MAINTAIN COOLER BELOW 40F. CRITICAL VIOLATION 7-38-005A.
All cold food shall be stored at a temperature of 40F or less. OBSERVED POTENTIALLY HAZARDOUS FOODS AT IMPROPER TEMPERATURES. OBSERVED COOKED EEL STORED ON TOP OF THE TOASTER AT 65F. OBSERVED THE FOLLOWING FOODS STORED INSIDE OF THE SUSHI PREP COOLER: SUSHI AT 46F, CREAM CHEESE AT 45F, SCALLOPS AT 44F. MANAGEMENT VOLUNTARILY DISCARDED 14# OF FOOD WORTH $150. CRITICAL VIOLATION 7-38-005A.
