At time of unannounced routine inspection, observed 2 wait staff members, 2 employees engaged in food prep in the kitchen and 1 manager/person-in-charge (plus 1 dishwasher). Observed CFM testing flyer provided at last inspection with notes made by establishment. Observed certificate provided is operating manager at second location in Monticello. Manager/person-in-charge Jaime stated they were closed for 2 months and reopened 3 days prior to inspection. He stated he is working on getting the CFM certificate. This violation must be addressed by October 14, 2014. 5/7/14 **Warning** **Repeat Violation** Open callback 10/14/14: At time of callback, observed person-in-charge Jaime, plus one waitress, 3 employees in the kitchen and one dishwasher. Jaime stated he has attempted the CFM exam 2 times previously and failed. He stated they now have David Aranda Ocampo signed up to attempt the CFM exam on 10/30/14 and inspector observed confirmation letter of test date and name. Advised establishment to save email if Mr. Ocampo passes the exam. Time extension of 30 days granted. This violation must be addressed by November 14, 2014.
