  Clean pots, pans and food tray storage counters in the first floor storage room; mice feces and dead roaches also seen. Accumulation of old food debris and grease on stainless steel racks in same room. Dry Goods racks in basement has accumulation of food debris and rust. Racks inside the walkin refrigeration in the basement also rusted - repair or replace.

  Reach in refrigeration unit containing raw eggs recored at 58*f for undetermined duration, product destroyed at call and unit placed out of service pending repair or replacement.

  Mice feces observed on counters used for "clean" pots/pans and food trays on first level in storage area. Dead roaches observed in the same area on counters. Mice feces observed on the floor at service line ( unused at call ) on the left side of main service line. Mice feces observed on counters and floor area in basement dry goods storage and walkway leading to facility. Numerous fruit flies observed in the basement storage areas and adjacent to the walkin refrigeration/freezer units.

  All rusted counters and racks in storage room, dry goods and walkin refrigeration are required to be repaired or replaced.

  Employees toilet room in basement door held open and self closing device in disrepair. Repair or replace unit and keep the door closed at all time to prevent flying insect entry/exit as vector.

  Utilize the waste food disposal for all customer generated and kitchen generated food waste to eliminate vermin harborage/feeding sites.

  Clean the floors throughout the establishment with attention to corners and baseboards adjacent to and under equipment,counters and racks to remove observed grease, dirt and debris accumulations.
