Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
- Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER
Observation: Raw chicken is stored over raw beef in the walk-in cooler and the Traulsen cooler. The Traulsen cooler was rearranged during the inspection.
Correction: Prevent nesting of raw animal foods and store raw animal foods in order of descending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. Correct By: 11-Sep-2015

Observation: Sloppy joe meat in the walk-in cooler is not date marked.

Observation: A spray bottle of sanitizer is stored on the new shelf by the back door next to a bag of breading mix.
Correction: Rearrange area so that toxic materials are stored away from food, equipment and single service items by spacing or partitioning.
- PACKAGED AND UNPACKAGED FOOD - COVERED CONTAINERS OR WRAPPING
Observation: Sauces in the hot hold unit, multiple food items (potato pancake mix, chicken wings, raw beef, etc) in the Traulsen cooler were observed to be unwrapped or uncovered.
Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.
- EQUIPMENT AND UTENSILS - MATERIAL FOR CONSTRUCTION AND REPAIR
Observation: The new shelf by the back door is made of wood.
Correction: Remove or replace with a material that is corrosion resistant, durable, able to withstand repeated warewashing, smooth, easily cleaning, nonabsorbent, and resistant to pitting, scratching, scoring or decomposition such as heavy food grade plastic or stainless steel.
- NON-FOOD CONTACT SURFACES - CLEAN (repeated violation)
Observation: There is liquid accumulated in the bottom of the back bar beer cooler. There is food debris spilled on the bottom of the 2 door freezer. The ice machine shield and top of the bin closest to the door has mold. The ceiling of the walk-in cooler has mold.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The ceiling tile above the prep cooler and the ceiling tiles by the back door (above the new shelf) are pushed aside.
Correction: Replace the ceiling tiles so the ceilings are smooth and easily cleanable.
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: The ceiling light in the basement prep area is not shatter resistant.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
- CONTROLLING PESTS - INSPECTING PREMISE (repeated violation)
Observation: There are multiple flies in the kitchen, dining room and basement.
Correction: Routinely inspect facilities for insects and rodents.
- CLEANING, FREQUENCY AND RESTRICTIONS (repeated violation)
Observation: There is grease, food debris and other debris on the floor under the cooking equipment and prep cooler.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.

Observation: Shredded pork and Italian dressing in the bottom of the pizza prep cooler is cold held at 50 & 48°F. Raw hamburgers, Raw chicken, and cooked bacon in the 2 drawer cooler are being cold held at 55, 54 and 59.
Correction: Potentially hazardous foods were discarded from these coolers. Maintain cold potentially hazardous foods at or below 41°F. Foods may not be stored in these coolers until they are repaired/replaced. Correct By: 06-Aug-2014

Observation: There is liquid on the bottom of the back beer cooler in the bar with bottles of beer in it. There is liquid on the bottom of the 2 door freezer. The floor, walls and ceiling in the walk-in cooler have food debris and mold on them.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Correct By: 06-Aug-2014

Observation: The wall covering is separating from the wall by the 3 compartment sink/4 burner stove providing a gap for debris to accumulate.

Observation: There are multiple flies in the kitchen, bar, dining room and basement. Fruit flies are also noted in the bar area. Mouse droppings were noted on the top shelf of the shelving unit in the basement storage area.
