Observation: Large food bins are cracked and repaired with tape.
Correction: Provide food contact surfaces that are smooth, free of sharp edges and are free from breaks and cracks.
- WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES (repeated violation)
Observation: An iodine test kit is not available for checking sanitizer concentrations.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
- FOOD CONTAMINATION PREVENTED - MISCELLANEOUS CONTAMINATION
Observation: Bread ends are stored in plastic to-go bags that are not food grade.
Correction: Change methods or procedures to protect foods from contamination.
- THERMOMETERS - FUNCTIONALITY - INTEGRAL OR PERMANENTLY AFFIXED MEASURING DEVICE (repeated violation)
Observation: Prep coolers are not supplied with a thermometer.
Correction: Coolers shall be equipped with a thermometer.
- UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
Observation: Pizza boxes are stored on the floor in the upstairs storage area.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.
- UTENSILS, EQUIPMENT AND LINENS - PRESET TABLEWARE
Observation: The preset tableware is not properly covered to prevent contamination of food contact surface.
Correction: If preset, utensils shall be protected from contamination by being wrapped, covered or inverted. Any exposed, unused settings shall be removed when the consumer is seated or unused utensils shall be removed after the consumer is seated and be properly cleaned and sanitized.
- TOILET FACILITIES - RECEPTACLES - COVERED
Observation: Unisex employee bathroom is not provided with a covered waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.
- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: The new salad prep area has raw wood floors, walls are currently insulation covered in plastic and ceiling is not finished. Paint is peeling on the walls by the stairs off of the kitchen. Flooring upstairs in the storage area are peeling and taped. Shelving in the upstairs storage area is raw wood and needs to be finished.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- LIGHT BULBS - PROTECTIVE SHIELDING
Observation: Lights located in new salad prep area are not shatter resistant.
Correction: Provide shielded, coated, or otherwise shatter resistant light bulbs.
- TOILET FACILITIES - ENCLOSED
Observation: Employee restroom is not provided with a tight fitting self-closing door.
Correction: Provide a self- closing door mechanism on bathroom door to provide a tight fitting self closing door.

- FLOORS, WALLS AND CEILINGS - CLEANABILITY
Observation: There is a gap between the wall and ceiling covering around the kitchen and above the 3 compartment kitchen sink. The corner piece by the dish machine is missing exposing plywood. There is a gap in the corner where the 2 walls meet between the 3 compartment sink and the dish machine.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
