Critical: PHF/TCS, COLD HOLDING (corrected on site) Observation: Dry batters used to bread/batter raw meats (chicken, calamari, fish) were being held at room temperature. Items were datemarked. Dry breadings that come in contact with raw animal food products shall follow the 7 days of datemarking, be sifted, and remain at cold holding of 41F or below. Manager discarded dry breadings at the time of inspection.
Correction: Maintain cold potentially hazardous foods at or below 41°F
Critical: DATE MARKING - DISPOSITION (corrected on site) Observation: Sliced deli ham in the True Upright cooler in the grill area downstairs had exceeded its date mark. Food item was date-marked as being prepared on 9/17/15. Manager voluntarily discarded product during inspection after being discussed.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked.
APPROVED PROCEDURES - ROP - HACCP PLAN Observation: Reduced oxygen packaging used for cooked and some raw meats does not have an approved HACCP plan. Manager stated ROP will be stopped and a HACCP plan will not be submitted at this time.
Correction: Stop using reduced oxygen packaging until an approved HACCP plan is in place.
CONSUMER ADVISORY Observation: Consumer advisory is stated on the meenu although food items that apply to the consumer advisory are not denoted.
Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
WAREWASHING - SANITIZING SOLUTION - TESTING DEVICES Observation: An Iodine test kit is not available for checking sanitizer concentrations for both iodine glass washers that are used in both bar areas in the facility.
Correction: Provide a test kit or other device for measuring the concentration of sanitizing solutions.
FOOD CONTACT SURFACES - SOILED Observation: Industrial can opener and interior of ice machine has visible old food debris and other substances which could be potential contaminants.
Correction: Maintain food contact surfaces in a clean condition.

Critical: PACKAGED AND UNPACKAGED FOOD - NESTING OR RAW ANIMAL FOODS STORED TOGETHER Observation: Raw Beef is being stored above Fish and Lobster tails in Walk in Cooler.
Correction: Prevent nesting of raw animal foods and store raw animal foods in order of decending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. Store Fish above Beef, Pork, and Chicken
USING A HANDWASHING SINK - ACCESSIBLE Observation: According to head cook, handwash sink is being used for prep as well. Provide a food prep sink with an one inch air gap that is separate from hand wash sink.
Correction: Keep handwashing sink free of obstructions and in working condition at all times to allow for frequent handwashing.
FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR Observation: Multiple food items are stored on the floor in the walk in freezer. And a couple bulk items in walk in cooler.
Correction: Store all food items 6 inches above the floor.
TOILET FACILITIES - RECEPTACLES - COVERED Observation: Female bathroom is not provided with a covered waste receptacle.
Correction: Provide a covered waste receptacle in female bathroom for use with sanitary napkins.
