Observation: The small shelf between the prep sink/drainboard and the cabinet in front of the stove are both worn and uncleanable.
Correction: Replace these surfaces with approved, smooth, cleanable material.

Observation: The paper towel dispenser at the kitchen hand wash sink is not dispensing paper towel properly.
Correction: Repair paper towel dispenser.

Observation: The door gaskets on the 4 door and 2 door cooler in the kitchen are soiled/torn and uncleanable.
Correction: Replace the door gaskets on these two coolers.

Observation: Some blue coated shelves inside coolers have the coating worn off and metal is rusting.
Correction: Discard or replace equipment and utensils that are not: corrosion resistant, durable, able to withstand repeated warewashing, smooth, easily cleaning, nonabsorbent, and resistant to pitting, scratching, scoring or decomposition.
- NON-FOOD CONTACT SURFACES - CLEAN
Observation: Non-food contact surfaces of fryers (including sides, legs, and wheels) are soiled with grease buildup.
The storage cabinet under toaster is soiled with dirt buildup.
Cabinet surface below trash can in toaster area is damaged.
Base of large can opener is soiled.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
Replace or repair cabinet surface below trach can.

- THERMOMETERS - AMBIENT AIR AND WATER - ACCURACY
Observation: Digital readout thermometer on #8 Delfield cooler is flashing a wide range of inaccurate temperatures.
Correction: Calibrate or replace thermometer. Be sure all coolers and freezers have accurate thermometers.
- POSTING OF CERTIFICATE
Observation: There is no State of Wisconsin food manager’s certificate posted in the food establishment.
Correction: Provide & post an original State of Wisconsin food manager's certificate. There is a question of whether there is anyone that has a current certification. None found within the State database. If no one is current (and that person must actually work in this establishment) then an employee must register for the class within 2 weeks. Call with registration information.
- IN-USE UTENSILS, BETWEEN USE STORAGE
Observation: Scoop in spice was improperly stored with the handle lying in the spices.
Solo cup used to dispense onions.
Correction: Store in-use utensils in the food item with handle extended.
Use only handled scoops for dispensing food.
- FOOD CONTAMINATION PREVENTED - MISCELLANEOUS CONTAMINATION
Observation: Ketchup is subject to potential contamination by customers when ketchup bottle contents are being combined.
Correction: Change methods or procedures to protect foods from contamination. Do not combine partially filled ketchup bottles. Empty bottles completely. Wash, rinse and sanitize bottles before refilling with fresh ketchup.
