Observation: The following noted in need of cleaning:
-Kitchen hand sink
Paper towel dispenser in kitchen
-Dish trays
-Inside and outside of dish machine
-Dish machine floor drain
-Outside of salad bar cabinets
-Salad bar containers stored in salad bar cooler
-Soda machine and iced tea nozzles and ice chutes
Correction: Maintain food contact surfaces in a clean condition.
- INSECTS AND RODENTS - OUTER OPENINGS - SOLID, SELF-CLOSING AND TIGHT FITTING DOORS (repeated violation)
Observation: This is a repeat violation. The exterior door near the mop sink is not tight-fitting which could allow entrance of insects or other pests.
Correction: Make the door self-closing and tight fitting to prevent the entrance of insects or rodents.

Observation: Employee observed handling cooked pizza and garlic bread with bare hands.

Observation: Chlorine concentration in dish machine measured 0 ppm. Sanitizing solution bottle was empty. Sanitizing solution was replaced and primed during inspection. Chlorine concentration still measured 0ppm. Discontinue use of dish machine until it can maintain 50 ppm chlorine
Correction: Provide training to employees on proper use and concentration of sanitizer or repair or adjust warewashing equipment to provide proper sanitizer concentration. Correct By: 03-Sep-2015

Observation: Cutting or piercing part on can opener is dull.
Correction: Replace cutting or piercing part to reduce the hazard of metal fragments caused by dull or worn parts.

- Critical: PHF/TCS, COLD HOLDING (corrected on site)
Observation: Sausage (47F), Pepperoni (49F), Cheese (50F) in the Main Line Prep Cooler were cold held above 41°F. Manager moved all food from the Main Line Prep Cooler to the Walk-In Cooler (33F). Higher temperatures were only noted in the top/open portion of this cooler. Food may be higher than 41F because it was not covered.
Correction: Use covers and/or ice pillows to help maintain cold potentially hazardous foods at or below 41°F
- EQUIPMENT AND UTENSILS - GOOD REPAIR
Observation: Pre-wash sink faucet is leaking. Replace, repair, and/or clean caulking by pre-wash section of dish line.
Correction: Repair equipment to good condition.
- NON-FOOD CONTACT SURFACES - CLEAN
Observation: The following noted in need of cleaning:
-Dish Machine Floor Drain
-Dish Machine Inside
-Soda and Iced Tea machine nozzles and behind the nozzles
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: There is a gap between the door and threshold of the back emergency exit door which could allow the entrance of insects or rodents.
Correction: Repair this hole or gap so that entrance by insects or rodents into the food establishment is prevented.
