Observation: Raw ground meat is being stored over the top of fish in the prep cooler and in the walk in cooler. Foods were covered and/or packaged, no evidence of cross contamination noted.
Correction: Store raw animal foods in order of decending hazard to prevent cross contamination. Separate raw animal food species from one another except when combined as ingredients. The operator separated the items at the time of this inspection.

Observation: Ice making water is coming into contact with a heavy amount of mold on the interior surfaces of the ice making machine.
Correction: Empty, disassemble, clean and sanitize the ice machine according to the manufacturer's instructions. The ice machine must be cleaned and sanitized often enough to keep it free of mold. Correct By: 05-Nov-2015

Observation: Soda beverages are coming into contact with slime mold on the inside of the soda gun (at the waitstaff beverage station).
Correction: Clean and sanitize the soda guns and drip cups often enough to keep them free of mold. Use a light to see if its has been thoroughly cleaned. Correct By: 05-Nov-2015

Observation: An opened factory package of ham and imitation crab meat were not dated with a seven day expiration. Pre-portioned packages of cooked meat were dated 10/18. The operator stated these items were taken out the freezer on Monday (two days ago). The date mark did not indicate the time the packages were in the freezer.
Correction: Once factory packages of ready to eat meats and seafood are opened they must be dated for a seven day expiration. If ready to eat items are subsequently frozen a system to keep track of the time the product is frozen is required. The operator correctly dated the packages at the time of this inspection.

Observation: Guacamole has exceeded its date mark (10/23).
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked. The operator discarded this item at the time of this inspection.

Observation: A sauce is being stored in its opened can (in the cooler with the glass doors in the middle prep area). The can is rusted.
Correction: Do not store foods in their opened cans. Transfer the foods to a food grade plastic or stainless steel container. This item must be discarded. Correct By: 04-Nov-2015

Observation: A hose is connected to the mop sink faucet. The mop sink faucet has an unapproved backflow prevention device integral to the faucet.

Observation: The consumer advisory on the menu does not indicate what foods are served undercooked. Hamburgers are cooked to the consumers liking.
Correction: Provide a consumer advisory that discloses which items are undercooked and reminds the consumer that eating the undercooked item increases the health risk. For example, place an asterisk next to the hamburger items on the menu that leads to a footnote that states "Hamburgers ordered rare, medium rare, or pink in the middle might be undercooked. Whether dining out or preparing food at home, consuming undercooked meat, poultry, fish, seafood, or eggs may increase your risk of foodeborne illness." Correct By: 04-Dec-2015

Observation: Water is dripping from the bracket area of the walk in cooler evaporator. The drips are falling onto pails of food below.

Observation: Light fixture in the kitchen are missing guards (plastic tubes) or are missing the end caps that keep broken glass in the tube.

Observation: The doors in the kitchen that open to the outside are not tight fitting and do not have weatherstripping that is effective at keeping out insects.
