**CDC Risk Factor Violation 14 F**: 4-601.11 Equipment, Food-Contact Surfaces, Nonfood-Contact Surfaces, and Utensils.
• “Clean” dishware in ware washing area stacked wet, noted food debris on baking pans. Some baking pans were still greasy.
• Clean and maintain. Do not stack dishware wet
(A) EQUIPMENT FOOD-CONTACT SURFACES and UTENSILS shall be clean to sight and touch.
(B) The FOOD-CONTACT SURFACES of cooking EQUIPMENT and pans shall be kept free of
encrusted grease deposits and other soil accumulations.
(C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of
dust, dirt, FOOD residue, and other debris.
**Critical Violation 45 C**: 4-101.11 Characteristics.
• White containers noted as stained and in ill repair.
• Red containers for sauces (end of oven), noted as soiled and in ill repair.
• Wall in dish area has holes in stainless steel. Repair.
• (this could potentially serve as a hiding place for vermin/pests).
Materials that are used in the construction of UTENSILS and FOOD-CONTACT SURFACES of EQUIPMENT may not allow the migration of deleterious
substances or impart colors, odors, or
tastes to FOOD and under normal use conditions shall be:
(A) Safe;
(B) Durable, CORROSION-RESISTANT, and nonabsorbent;
(C) Sufficient in weight and thickness to withstand repeated WAREWASHING;
(D) Finished to have a SMOOTH, EASILY CLEANABLE surface; and
(E) Resistant to pitting, chipping, crazing, scratching, scoring, distortion, and decomposition.
4-205.11 Food Equipment, Certification and Classification.
FOOD EQUIPMENT and UTENSILS for use in FOOD ESTABLISHMENTS shall meet the published standards for sanitation of an American National Standards
Institute (ANSI)-accredited EQUIPMENT CERTIFICATION PROGRAM, or the APPROVAL of the department of health and family services and the department
of agriculture, trade and consumer protection.
Violation 47 A**: 4-601.11 Equipment, Nonfood-Contact Surfaces
• The following items were noted as soiled and need to be cleaned and maintained clean:
o Handles of equipment (retarder, proofer, coolers, freezers, etc); be sure to clean the under portions of these units.
o Handle of the spray hoses
o Fans in the dish area and in walk-in cooler
o Floor in walk-in cooler, under shelving
o Shelving area at the end of the oven/cutting area; pay attention to the under portions of the shelving as well.
o
(C) NonFOOD-CONTACT SURFACES of EQUIPMENT shall be kept free of an accumulation of
dust, dirt, FOOD residue, and other debris.
Complaint received in office on 9-25-2009. Complainant stated that her and her family ate at the establishment on September 17. They put
parmesan cheese on their pizza and there were little gnats in the cheese. The cheese shakers were on every table.
Spoke with manager. She was aware of the complaint. She stated that there was one gnat in the cheese. Am told that the shakers are cleaned out
weekly. Batzner pest control is pest company. No receipts were available. Looked at the paremesean cheese shakers, did not observe any gnats
in them. Did not observe any flies/gnats in establishment. Cheese comes in bags in a box. Stored in walk-in cooler until used on tables.
Low temperature dish machine, 200+ppm chlorine sanitizer + pre-wash sink + separate hand wash sinks + mop/utility sink.

This is a follow up to the routine inspection done on 10-5-2009.
The following violations were corrected:
14F) Food contact surfaces are now clean and dishware is stored dry.
45C) New white containers ordered; red containers now clean and in good repair; wall in dish area was temporarily repaired. Holes were filled
in with caulk. Need a more permanent fix, ie replacing the panel, etc.
47A) Non-food contact surfaces are now clean. Maintain.
Violation 42 E: 4-901.11 Equipment and Utensils, Air-Drying Required.
Some of the clean dishware was stacked wet.
After cleaning and SANITIZING, EQUIPMENT and UTENSILS:
(A) Shall be air-dried or used after adequate draining, before contact with FOOD; and
(B) May not be cloth dried except that UTENSILS that have been air-dried may be polished
with cloths that are maintained clean and dry.
Two handwash stations available with soap, paper towels, and gloves. Low-temperature dishwasher with sodium hypochlorite concentration
between 100-200 ppm. Wet wiping cloths stored in buckets with chlorine based sanitizer >100 ppm. Test strips are available. Datemarking
system present upon opening all containers and/or thawing of potentially hazardous foods. Written employee health policy in place for all
employees.
