- 0840 - Critical The prepared ready-to-eat (RTE) FOOD (2 containers of meat sauce and marinara sauce) was not discarded by the ""consume by"" date.
- 1790 - Repeat The inside cavity of the small GE microwave oven had accumulation of food debris.
- 2000 - Coffee filters were left out unprotected and subject to contamination.
- 1570 - The following EQUIPMENT was observed in a condition that prevents necessary maintenance and easy cleaning.1. Hobart 4 door refrigeration rusting rusting inside in several different locations; rollers on this unit are worn and broken.2. Shelves storing stainless steel coffee creamers are not sealed, are absorbent, and are worn on the edges.
- 1770 - Encrusted grease deposits found on food contact surfaces of many sheet pans. Pans were not in use.
- 3200 - Intake and exhaust air ducts are not being cleaned in Men and Women's restrooms. Both have dust accumulation.
- 3170 - The following Physical structure is not maintained in good repair:1. Wood wall behind dry storage of can goods has open seams and some raw wood surfaces.
- 3180 - 1. Dust accumulation on fans and light fixtures in dinning area.2. Food debris, trash, etc. under most equipment in kitchen.

Coving along side wall of walk-in refrigerator has caulking missing and separating.3. Wall behind the canned goods and other dry storage area is not easily cleanable and has crevices that need to be caulked. Wall needs repainting.
