• 0380 - Critical Food from damaged packaging offered for sale or service. Noted one dented can (bamboo) . Dent on the seam . owners to set up dented can section

• 0470 A 4 - Corrected During Inspection Critical Observed the following food was observed stored without being in packages, in covered containers, or wrapped: in the walk in e.g Container stored over uncoverd egg rolls.
  Store food in packages, covered containers, or wrappings.
• 1700 - Corrected During Inspection Critical wash rinse and sanitize was set up wrong. wash was in the middle.
  reset to wash rinse and sanitize.
• 1770 A - Corrected During Inspection Critical Repeat The following utensils were observed soiled to sight and touch: some knifes stored on the magnet were dirty/greasy. .
  Clean and sanitize these surfaces for food contact.
• 0820 A 2 - Corrected During Inspection Critical shelled eggs, cold holding at improper temperatures. was 63 sitting out after lunch. moved to sandwich unit.
  Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F or below.
• 0610 - Food stored on the floor or food stored less than 6" above the floor in the walk in freezer. Note they were in boxes but stored on the floor.
  Elevate food storage onto approved shelving with minimum 6" legs or casters.

March 25, 2009 (Routine)

• 0130 - Corrected During Inspection Critical Food employees observed with soiled hands and arms. No employees washed arms till brought to their attention.
  Inform all food employees to keep their hands and exposed portions of their arms clean.
• 1770 A - Corrected During Inspection Critical The following utensils were observed soiled to sight and touch: a) knifes on the magnet. b) pans/pots on shelf above 3 comp sink had a greasy feel. looks like the 3 comp sink was set up wrong wash in middle basin and sanitize which probably caused the pans to not cleaned properly.
  Clean and sanitize these surfaces for food contact.
• 2930 - Screen door needs to be self closing.
  repair the hinges so that the door will close.
• 1060 - The nonfood contact surface of the wooden shelfs is not corrosion resistant, nonabsorbent, and/or smooth.
  Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent. Need metal shelves.
• 1800 - These nonfood contact surface of the has accumulations of grime and debris. a) Stain on wooden shelves. b) Magnet with wood in th has food particles on it and is what is making the knifes dirty. c)Bottem shelf near sandwich unit needs cleaning.
  Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Management not aware of employee health policy. Showed managers the written forms in their files and asked them to correct as soon as possible..
  Have the license holder require employees or applicants report to the person in charge if they meet one or more of the following high-risk conditions: Is suspected of causing, or being exposed to, a confirmed disease outbreak caused by S. Typhi, Shigella
• 1770 A - Corrected During Inspection Critical The following utensils were observed soiled to sight and touch: some pots and pans on shelf above 3 comp sink. Some knifes stored in a magnet. rewash started .

• 0820 A 2 - Corrected During Inspection Critical noted egg roll at 45 on the top layer int he the sandwich unit. Bottom layers 41 F. discarded top egg rolls. B) noted one raw burger patty at 46F. Discarded. Other patties were at 40 F. cold holding at improper temperatures
  Relocate food to a refrigeration unit that is capable of maintaining food storage at 41F or below.
• 0930 - Corrected During Inspection Critical Noted consumer advisory was not next to the raw food in the menu. No star next to the raw food. Sign put up next to sushi station.

• 3340 - Corrected During Inspection Critical Container of a ant insecticide are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Noted that it was stored on the soap dispenser near the sushi section. removed and put on the floor where it belongs. Handwash station was also cleaned.
