- Found open bag of raw chicken uncovered in a walkin cooler. Also, found a container of cole slaw uncovered in another walkin cooler.
- Found black mold-like substance along backsplash of 3 comp sink. Clean & calk this.
- Found grease/scum/food debris on some kitchen equipment/shelves/containers.

- The counters in the drive through area are damaged and have exposed wood underneath. These areas need to be sealed. The vent on the front of the same unit is rusted. There is a drip in the lower left corner under the ice well in the drive through area. I pointed it out to the manager. We were unable to check the ice storage due to the cover being screwed on. The labels for the Henny Penny on the left and the sifters are coming off.
- There is buildup accumulated throughout the facility on almost all surfaces. For example: holder for can opener, racks, shelves, lid holder for drinks in dining area, prep areas, handles, fryer stations, sifter station, and mixer. The door to the chicken walk in cooler is caked with buildup.
- The floors throughout the facility have buildup on them and have standing water in several places.
- There are damaged floor tiles throughout the facility as well as missing or damaged coving. I pointed these areas out to the manager.
- There is food buildup and splatter on walls throughout the kitchen.
- There are damaged ceiling tiles by the hot holding area. I pointed this out to management.
- There were boxes of gloves stored with chemical storage. They were moved.

- Ice scoop was found with the handle down in the ice in the ice machine.
- Observed a couple employees only washing their hands for approximately 10 seconds. Retrained employees on site. Conduct a training with all staff on proper hand washing procedures and submit documentation to TCHD, Attn: Kim by 10-9-14. Include a statement that the manager has observed staff properly washing their hands as part of the training.
- Hot holding cabinets, fryers, cook line had a build up of food and grease. Clean and sanitize these items.
- Insides of stainless fridges and counter tops had a build up of food debris. Clean and sanitize it.
- Outsides of biscuit warming drawers, cooler and freezer door handles, hot holding cabinets, water heater, water pressure tank, and shelves had a build up of debris. Clean and sanitize these areas.
- Found build-up of debris on floors. Clean the floors.
- Found damaged/missing floor tiles. Replace them.
- Walls and ceilings had a build up of dust, grease, and food debris throughout the kitchen. Clean the walls and ceilings.
