- Found gravy in the hot holding unit at 112F. Hot food must be held at 135F or above. Corrected--It was discarded by the manager.
- Observed several front line employees touching their faces and not washing their hands. Employees were retrained by inspector and correctly washed their hands during the inspection. Manager must retrain all employees on when, how, and why to wash their hands and submit documentation to TCHD, Attn: Kim by 3/10/16.
- Observed employees not wearing effective hair restraints on the front line and drive thru areas. Employees working with or around food must wear hair restraints.
- One employee in drive thru had a lot of debris on the front of her shirt.
- The wire shelf with single use paper products stored on it right outside of the walk in freezer had was rusty. Clean and repaint it or replace it.
- The burger freezer seals and outside of the rolling cart under the prep sink had a build up of debris. Clean and sanitize these areas.
- Repair/seal the holes in the walls in the walk in cooler and above the hand sink next to the ice machine.

- Observed a box of single service items on the floor in the storage area. All food service items must be stored at least six inches off the ground at all times.
- Observed employee handling clean dishes after washing dirty dishes. This employee did not wash hands between tasks. Spoke to staff regarding when to hand wash and requested that the employee washed hands before handling clean dishes.
- Observed cardboard being used on one of the shelves in the walk in cooler. Cardboard is not durable or easily cleanable. Remove.
- Observed missing seal around handsink near the cooking area. Re-seal. Observed rusty shelf near the walk in freezer. Repair or replace. Observed dirty broken seal on the cooler next to the fryer. Clean and repair seal.
- Observed debris accumulation on the cabinets in the cake decorating area, the freezer next to the frying station, and the two freezers next to the grill. Clean.
- Observed a hole in the wall above the hand sink near the register. Repair.
- Observed dust accumulation on the vent, wall, and ceiling in kitchen area. Clean.

- Observed green tape on the reach in deep freezer near the back door and to one of the reach in prep coolers on the front line. Tape is not a smooth & easily cleanable surface. Remove tape. Also, the handle to the deep freezer was broken. Repair handle.
- Wash, rinse and sanitize the mounted can opener on the cake prep counter, observed food debris build-up.
