Say you get an offer from an out of state company. Neither is the work, location, or salary your first choice. They want an answer in a very short time frame and at the same time, you're interviewing with your dream company in your town. The out of state company is completely inflexible with extending your deadline for accepting the offer. You accept the offer from the out of state company and then your dream company in your town makes you an offer than you can't pass up.
Would you renege the out of state company, especially if they don't have a presence in your local area and it's in a town you probably will never consider moving to in the future?
Depends. What space are you in? Are there lots of opportunities or few of them? Does everyone know everyone?
In my universe everyone knows everyone. I have heard about people interviewing for jobs from 3 or 4 other people in my personal network, and heard about when they have accepted or withdrawn.
One time I shared a story about a horrible candidate I interviews and 2 other colleagues had come across the same guy - I didn't share anything incriminating or personally identifiable, but they knew.
I got pinged by a hiring manager, informally, about a job, and I had a quick chat about it. Turns out the hiring manager knew the person who hired me and had coffee with her the week before. She was mad when I told her about it (I had only mentioned I referred a friend).
It is not uncommon in my industry to work with the same people multiple times. So word definitely gets around.
If the world is small, don't renege. If you are let's say and accountant where you can get job in lots of industries and are unlikely to run into the same hiring manager, it is less of a worry.

I would never dismiss my dream job offer. Will this make the other company cry? I'd send them a hankie and laugh all the way to the bank.
