I have an interview at a place that 'always' seems to be hiring. I've been here 5 months, and they have had 3 or for ads.

It's not that they are expanding (so I can tell) but that there seems to be a lot of turn over. Again, this is in an industry that traditionally pays poorly, expects you to be Pollyanna Sunshine the whole day, and most people 'could not handle it.' The turnover rate is very high.

Do I go and ask the woman "why are you now looking for 3 people at once?" Does this seem like a red flag?

I really need a job but I don't want to wind up in another crappy organization.
Any opinions?

You can't take anyone's word for it, as it may be the right job for you. Look at the trucking industry, bartenders, military... Some of these companies have 100% turnover annually. It's your right to interview the employer as to security and advancement. I would think that your asking shows interest.

Anywhere you have low pay you will have high turnover, people have zero incentive to stay. It's only a red flag if you have high-paying jobs with high turnover.

Google the business. Usually if a place is a bad place to work, employees will often vent on the internet about it.

I'd ask them why they have had several ads running and if they have a lot of turnover and see what they say. I always ask about turnover in interviews anyway.
