Looking for an example about incompetence? How about your boss who treated this employee poorly?

What you described about the coworker is not incompetence, it is cultural gap. It was mean how your boss told her about spelling out DOUG. Its his job to be polite, not sarcastic. She is obviously nervous & making many mistakes in the process. Usually the managers work with the HR to setup some training sessions for the employees who need improvement in certain areas. So she wasn't incompetent, but your boss was out of line & failed to do his job.

I don't see the 3 examples you listed as incompetence really but some sort of transition to a new culture.

I don't really care to share examples of incompetence. Also, a "puzzled facial expression" could be something else. You don't know.

What's the point?

I have never really been able to put my finger on the line between incompetence and being human. I mean its pretty easy to tell the extremes but not when your close to the interface.
