Ok so I Goolged "DOT physical" and there is a document in the first link that gives you exactly what is tested. Drugs are not on the list - your urine is checked to indicate any issues (diabetes I would assume would be a major one probably). There is a section where the doc can check if you "look" like you are a drug user.
Typically drug testing is the responsibility of the employer.
Everyone kinda jumped on the bandwagon here with the whole urine testing thing...but I do not believe it is accurate. My guess is it's very similar to FAA medicals which check urine as well. Not that any of this should make a difference, of course, but accurate info is better than bad info.
On a side note why the OP didn't just google it like I did...instead of asking on a forum where he/she would inevitably get bad info...are we really that lazy nowadays?
Our company does drug screens on all "new hire" and pre-employment DOT physicals. Then for annual physicals it is up to the patient's employer to order a drug screen along with the physical. Some order it but most do not unless they have reasonable suspicion.
Hair follicle screening is not allowed for DOT but many other companies request it. It does give results for a longer time frame than urine drug screens.
The DOT drug screen checks for 5 drugs, and also the urinalysis checks for sugar, blood, protein and a few other things.

OK what I'm saying is that the employer is ordering that test. The specific DOT test DOES NOT test for drugs. Show me on form 649-F where urine drug results are supplied? Hint, there is no section to report that.

Are you with Quest? Quest does all of ours IIRC.
No, I'm not with Quest, our company used several different labs, depending on which of several drug screen panels we are using. For our DOT drug (5-panel) screens we use MedTox. We use LabCorp for our 10-panels, and other labs for 9-panel and 9+oxy/opi, depending on the preference of the employer.
We use Omega for hair follicle testing, although this is only non-DOT.
