That would be my guess. I'm from Queens, and it's probably a lot easier to social distance in the countryside than it is in a 40-unit apartment building. In many NYC apartment buildings, if you want to check your mail, you have to go to the lobby. If you want to wash your clothes, you have to go to the laundry room. If you want to do either of those, you have to use the elevator or stairwells. Etc.
There are other reasons available for the high hospitalization rates - I was just throwing one out (age). For instance, another is space and resources. Two people, same symptoms, same degree of "sick"- one might be told to "shelter in place" in NY - or told to head to the ER in another community. Lots of reasons. None of them sneaky.
They just discovered these infections at the Smithfield Factory last week, its a little premature to use current data. We know that the disease spreads very fast doubling ever few days and the governor didn't take any action.

Minnehaha county, which is the county that Sioux Falls is in, has a population of 193,000. As of today it has 1,157 diagnosed cases of COVID-19. That represents about 0.6% of the population. The real number is obviously much higher than that because of a shortage of tests but that gives it one of the higher rates of confirmed cases in the country. Given that Sioux Falls is in the early days of its outbreak those numbers will certainly go higher.
