Different boroughs? How about two different streets with the same name in the same borough!
Brooklyn has two Manhattan Avenues (Greenpoint & Sea Gate), and two Atlantic Avenues (Bed-Stuy and Sea Gate). There are also two completely different West 9 Streets.
But the all-city champion has to be Broadway--there's one in each borough.
There is a Linden Blvd in Brooklyn and 2 different Linden Blvds in Queens. I'm sure that they were all connected at one time, but now they are 3 disconnected streets with very different characters. For those of you who don't know:
Linden Blvd in Brooklyn runs from Flatbush Ave to Conduit Ave (it probably includes a little piece of Queens)
The lesser known Linden Blvd in Queens is from Cross Bay Blvd and Gold Rd to Peconic Street and Centerville Street
The better known Linden Blvd in Queens is from Rockaway Blvd to the Nassau County line (and becomes Central Ave).
