My employer pays $1680/month premium for my family HealthcarePPO plan. My paycheck portion deduction is $325/month.

$0 Deductible. Out of pocket max $6000/family. $20 co-pay. 100% coverage on in-network hospital visits, xray, preventative, physician service/surgical, maternity, and mental health.

Dental is by Delta Dental ($40 monthly family)

Prescription is covered by Medco($15/month). Vision is covered by Davis Vision ($13 monthly family).

My health insurance and benefit (life/paycheck); life (fixed amount) to cover anything if and when I moved on while paycheck is for getting a paycheck for 6 months in case I lose my job. Premium is around $60.00 per paycheck. Deductible is $2,000. $25.00 primary care visit. I only have medical insurance. My Care Credit card has zero balance, that is what I used for my Dental/Vision needs.
