Bass101, the info they ask is for the fingerprint card and isn;t being asked by employer. It is used in case any problems come up and to have a cross reference for your prints. If you dont complete it, the person taking the fingerprint will ask for it before signing the card.
Most of my jobs have involved working with/around children, so I've gotten used to the fingerprinting & background checks... they even required current TB tests for my last 2 positions. It does seem a little "Big Brother-ish," however I also understand the reason for these procedures. I guess it depends on your line of work, but I get why they need to verify you're not hiding anything - especially something like a sex crime, embezzlement, or anything else that could put the company at risk. I wouldn't quit a job over this, especially if you have a clean record!
In order to get a nursing license in most states we have to be finger printed and we have to PAY FOR IT OURSELVES!
My employers paid for the livescan (fingerprinting), but we had to pay for the TB test ourselves. I had private medical insurance at the time, which wouldn't cover "unnecessary" TB tests... also didn't qualify for a waiver at the free clinics (due to having insurance), so it ended up costing me $45. Totally lame catch-22.
