Outback does make a good burger. Other chains where I've had really good burgers include Logan's Roadhouse, the Green Iguana (Tampa area chain), Ruby Tuesdays, and Longhorn Steakhouse.
I make the best at home. Huge....disgustingly beautiful massive burgers, at least 3/4 pounders - 1 pounders, juice running down your wrists and arms burgers; gorgeous globs of heaven.
Fuddruckers for burgers out. I've heard Red Robin is good too but I've not been to one.
When I go out I like meat cooked a little more than I would at home. Usually I like it very rare but warm in the middle (at home). Simple reason is I don't know where the meat came from or what the kitchen may look like when eating out. At home I know where the burger came from (pasture around the house) and what my kitchen & grill look like.
Honestly, I think the Burger Bar has the best burgers anywhere. The kobe beef burger is just awesome. They are very expensive but well worth it. I have had burgers at most of the places mentioned and none of them are even close. I do prefer them medium-rare. I can not stand overcooked meat.

You know all this well done stuff because of ecoli...I remember eating hamburger meat RAW when i was a kid....if my mother was making meatballs I loved to take a pinch or two of it RAW..raw meat, raw eggs
and never got sick
Love a real juicy cheeseburger..you ask for it rare now same as steak and the waiter looks at you like you're nuts

This is the same reason that I usually order my meat cooked a step or two longer in a restaurant as well. Half the time you never get what you ask for anyways.
I usually only order medium rare at higher end restaurants that I know have high quality meat
