I have a couple of friends who have implants and are really happy with them. They've both said it's not a one-time expense, there's some follow up required over the years.
It must depend on the type you get, or maybe the surgeon. I know several women with them, none of them has had to have any "followup". This is 15 - 25 years down the road.
You describe the loss of shape due to the passing years and having children. It's possible that implants and other reconstructive surgery might help your appearance. But what I'm going to say is not so much directed at you, but to women in general, who might consider such a thing.
The most significant thing about implants, is that sooner or later, they'll have to come out, because of complications. What will be left, will not be pretty. The skin and other tissue will be stretched and deformed. It might be devastating to you and you'd rue the day you had it done.
Well, my opinion is.. Do what makes you happy. If you have the funds and the time, then why not? I don't know if you've researched it already, but I've read about breast implants out of sheer interest. The "teardrop" shaped implant is supposed to look more natural than the traditional round implants. This shape is becoming more popular because it doesn't have that "obviously fake" look. Something to look into? Also, like others have mentioned, finding a good surgeon is crucial. Don't just skip off to the local surgeon in your area, do research, read patient reviews- you might have to drive a few hours away from home to go to a top surgeon.
Her husband might be happy with them, but ultimately SHE has to be happy with them. They are part of her body, not just accessories for his pleasure.
That being said, OP, how are you set financially? Plenty of money set aside for retirement, kids college funds? If not, it should be a decision between you and your husband if this is the best use of your money. Assuming your money is pooled and not separate, his and hers.

There are exercises that firm the breast muscles to lift them. It might be worth
considering to see what changes you may get in just a few weeks. Surgery is
always an option.
I once heard a women talking on her cell phone in the hallway at a plastic surgeon's
office. She just had breast implants for her husband. She was not happy and in a lot of pain.
She told her friend (in pretty colorful language) that if she knew the pain that she was going
to go through she never would have agreed to it. She said she should have bought her husband magnifying
glasses so that her breasts would look bigger!!! Had to bite my tongue to not laugh at the last bit.
You might want to consider a breast LIFT. I had one. I never regretted it. There's a term for what you have, which is what I had but maybe not as bad as you describe yours. With a LIFT, they will be slightly smaller afterwards. I describe it as a pillow. Beforehand, the fabric on the pillow is all loose. Afterwards, seams have been taken in on the pillow fabric, so that the pillow is firmer and plumper and refreshed, but slightly smaller.
You can get both a LIFT with a small augmentation.
Warning: Don't do plastic surgery for someone else. Do it for yourself. YOU have to live with your body. Do what will make YOU happy.
Warning #2: If you get augmentation, don't go too large. That's the "biggest" mistake I see that women do. You want to look natural, not have a hard time finding clothes to fit. Breasts too big may also make you look overweight.
BTW, I tried exercises. They help w/the underlying muscles. But of course, there are no muscles in breasts. They will continue to sag. The skin on the breasts is stretched out. Only a lift, or filling them with something larger, will plump them up. If your nipples are now too low, they will stay low, unless you have a LIFT.

Good suggestions but it's possible she's tried that already. the sagging may be more due to the skins elasticity and firmness being broken down, and this has nothing to do with the chest muscles. In that case, like someone else said, implants are really the only way to go.

There are different types of implants though. Saline or silicone. saline feels and looks more natural.
