    Observation: Debris build-up on the non-food contact surfaces of the hot holding cabinets.
    Correction: Clean the hot holding cabinets as needed.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Several broken/missing floor & base board tile.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Repair floor & base board areas as needed.

    Observation: Observed accumulations of food residue on the following nonfood-food contact surface: the underside of the dining area customer self-service beverage station nozzles.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Physical Facilities in Good Repair
    Observation: Cracked floor tiles and ponding of standing water were observed at various locations throughout the facility. Also noted was greasy and muddy water behind the cookline fryers.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

  • Handwashing Signage/Handwashing Facilities (repeated violation)
    Observation: A sign or poster that notifies food employees to wash their hands is not provided at the women's rest-room.
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

  • Sanitizing Solutions, Testing Devices (corrected on site)
    Observation: There is no properly working test kit provided in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: OBTAINED AT TIME OF INSPECTION. Obtain a QUATNARY AMMONIUM sanitizer test kit to monitor sanitizer concentrations to ensure safe and effective levels.
  • Critical: Backflow Prevention, Air Gap* (corrected on site)
    Observation: An air gap between the water supply outlet and the flood rim level mop hose/mop sink basin is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
    Correction: DISCONNECTED. Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

    Observation: Observed accumulations of mold-like substance around the nonfood-food contact surfaces of the customer self-service beverage station nozzles.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Critical: Equipment and Utensils - Before Use After Cleaning*
    Observation: The female food worker was observed rinsing a sheet pan with a sprayer hose at the curbed mop sink, in the warewashing area, prior to re-use.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.
  • Hand Drying Provision
    Observation: No disposable towels were provided at the hand washing lavatory in the front service/drive-thru area.
