The purpose of this visit was a follow up to assess if the dish machine was sanitizing the food contact surfaces that had been washed and rinsed.

The purpose of this visit was to perform a routine inspection.
Please email me an invoice for the repair of the dishmachine by October 20, 2015. Additionally email a copy of your certified food manager food id card no later than November 9, 2015.

  • Critical: Chlorine Sanitizer Solution Concentration based on pH and Water Temp
    Observation: When tested, the low temperature, chemical sanitizing warewashing machine was observed with a sanitizing rinse concentration of less than 50 ppm chlorine solution.
    Correction: Immediately discontinue use of the dishmachine. Until the machine is adequately repaired, set up the three-compartment sink to manually wash, rinse and sanitize food-contact surfaces using a sanitizing solution at the proper concentration of at least 50 to 100 ppm total chlorine.
  • Critical: Food-Contact Surface Sanitized before Use after Cleaning (corrected on site)
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: pizza pans
    Correction: All utensils and food-contact surfaces of equipment shall be sanitized before use and after cleaning.SANITIZED IN THE 3-VAT SINK

    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448.ORS information given to PIC

    Observation: Observed that the area under the dishmachine is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

    Observation: Intake and exhaust air ducts are not being cleaned at a frequency necessary to prevent contamination by dust, dirt, and other materials.

    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (www.orsinteractive.com, Ph # 703-533-7600) within 10 business days. A copy of the photo identification card can be faxed to the food safety section at (703)653-9448 ATTN: A 1. Failure to provide a copy of the photo identification card will result in further enforcement action.
  • Physical Facilities Good Repair
    Observation: Observed that the caulking at the 3 VAT sink has visible mold and is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.ASKED MANAGER TO CLEAN AND CAULK THE SINK IN 10 DAYS.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information in a red folder labeled "Employee Health Policy.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Food Storage Containers, Identified with Common Name of Food (corrected on site)
    Observation: Unlabeled food containers with the following food items that are not easily identified by appearance: OBSERVED BOTTLES OF OIL THAT WERE NOT LABELED.
    Correction: ALL food and ingredients removed from their original container and stored in other food containers, squeeze bottles or "shakers" shall be labeled properly using the common name of the food (if the food or ingredient is not easily distinguished). For example, all cooking oils, salt, sugar, flour, starch, spices, herbs, etc. shall be labeled. CORRECTED: CFM LABELED THE ITEMS.
  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning:OBSERVED THAT THERE IS A GAP BETWEEN 3 VAT SINK AND WALL THAT NEEDS TO BE SEALED.
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Physical Facilities Good Repair
    Observation: Observed that the tubelight in the restroom needs to be replaced . Install a cover for the fuse/electrical box near the back of the kitchen area so that the exposed wires are covered.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the space between the 3 VAT sink and the wall has mold/mildew accumulation and is in need of cleaning.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

Me and I dine in at this Pizza Hut at this location and I ordered 8 wings, the employee ask me how do I want to wings hot or mild, I said I want half and half. She said they don't do half and half. I was thinking ever since I ordered at pizza Hut they always do half and half. What kind of service she's giving, telling lies. Or is this true?
