    Observation: Noodles, onions and peppers and roast beef cold holding at improper temperatures for less than four hours. Tomatoes out of temperature for more than four hourswhere voluntarily discarded from the right preptop. Milk in the bar cooler was cold holding at improper temperatures. Potentially hazardous food in the right preptop out for more than four hours were voluntarily discarded and food out of temperature for 2 hours were put in W.I.C.. Person in charge agreed to send inspector temperature logs for the next three weeks.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The door gasket to the reach in freezer is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
  • Equipment - Cutting Surfaces (repeated violation)
    Observation: The cutting board(s) along the prep tops are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: No detergent for the three compartment sink.
    Correction: In accordance with the manufacturer's label instructions, apply detergent or cleaning agent to wash water for effective cleansing of equipment and utensils prior to rinsing and sanitization.

    Observation: Blue cheese cold holding in left prep top for less than four hours. Sliced cheese in prep top left and cheese in the right prep top, milk in the bar refrigerator for more cold holding at improper temperatures for more than four hours.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Blue cheese was relocated to a refrigerator that is capable of holding 41 degrees or less. Sliced cheese in prep top left, cheese in right prep top and milk in bar refrigerator voluntarily discarded.

    Observation: Equipment food contact surfaces/utensils were not observed cleaned, wash, rinsed and sanitized after use. Accumulation of grime on slicer. Can opener heavy accumulation of grime.
    Correction: Wash, rinse and sanitize slicer and can opener.

    Observation: Soap was not provided at the hand washing lavatory in the kitchen by grill.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

    Observation: Walls behind reach in freezer, floors under storage racks in storage areas by stand up freezer and by the reach in freezer and floor by the grease trap, floor by drain, ceiling by the grill noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Correction: Separate types of raw animal foods from each other such as beef, fish, lamb pork, and poultry during storage, preparation, holding, and display by the following: (a) Using separate equipment for each type. (b) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. (c) Preparing each type of food at different times or in separate areas.

    Observation: Dispensing utensils improperly stored between uses. Ice scoop handle in ice.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.
