  • Toilet Rooms - Enclosed (corrected on site) (repeated violation)
    Observation: Toilet room is not provided with a self-closing door. Its door is standing open to adjacent room where are the lockers, laundry, and mop sink, and door from that room into kitchen is propped open.
    Correction: Provide a self-closer for the toilet room door. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas. Correcting the condition by closing the door between the outer room (lockers/mop/laundry) and the kitchen is acceptable only as a temporary measure until self-closing device or hinges shall have been installed on the toilet room door.

NOTE: No insects were observed during this inspection. . . . . . SUGGESTION: if it is impossible to acquire a self-closure device for this restroom door, consider removing the self-closure device from the outer door to the laundry/locker room, and putting it on the employee restroom door instead.

Until the restroom door is equipped with a self-closure device or self-closing hinges, it is acceptable to keep the outer (laundry/locker room) door closed, provided that this door is always closed whenever the facility is in operation (except during maintenance and cleaning). (This observation was corrected during inspection by closing the door to the laundry/locker room.)

    Observation: In use open drinking containers stored in a manner that may contaminate food, food contact surfaces or utensils.
    Correction: Employees may drink from a closed beverage container if the container is handled to prevent contamination of (1) The employee's hands (2) The container and (3) Exposed food clean equipment, utensils, and linens and unwrapped single-service and single-use articles.
  • Critical: Package Integrity* (corrected on site)
    Observation: Food packages are not in good condition or the integrity of the package contents are not protected from adulteration or potential contaminants (ex: dented food cans).
    Correction: Discard, or return to distributor, any food packages that are not in good condition or the integrity of the package contents are not protected from adulteration or potential contaminants. (The dented cans were separated from the others and set aside for return to distributor.)
  • Single-Service and Single-Use Articles, Use Limitation
    Observation: Manufacturer container (formerly containing "party punch" concentrate) was observed reused for the storage of potato salad.
    Correction: Discontinue the reuse of manufacturer containers for food storage. Provide approved reusable food storage containers designed for your food storage needs.
  • Critical: Equipment and Utensils - Before Use After Cleaning* (corrected on site)
    Observation: The food-contact surfaces of the following equipment were not observed sanitized: cookware items in the sanitizer vat of the 3-vat sink. The items had been placed in the vat to soak, in accordance with the manufacturer's directions, but some were not fully immersed and so were not exposed to the sanitizer for adequate contact time. The sink was over-crowded and not all of the items placed in it could be submerged simultaneously.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.
  • Handwashing Signage/Handwashing Facilities
    Observation: A sign or poster that notifies food employees to wash their hands is not provided at all lavatories used by food employees. NOTE: Employees were observed properly washing their hands. The issue is only that signage has still not been posted, as required by the Regulations.
    Correction: Provide a sign or poster that notifies food employees to wash their hands at all handwashing lavatories used by food employees.

  • Food Storage Containers - Identified with Common Name of Food
    Observation: Unlabeled food containers (wheeled bulk-storage bins in bakery area, for flour, sugar, etc.).
    Correction: Label working containers with the common name of their contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Equipment and Utensils - Multiuse, Characteristics
    Observation: The food contact surface of assorted pots and pans is not (or is no longer, due to use) durable, nonabsorbent, easily cleanable, resistant to pitting. Observed heavily pitted interior surfaces on assorted pots and pans.
    Correction: Repair or replace food-contact equipment to permit easy cleaning and prevent the migration of deleterious substances, or transference of colors, odors, or tastes to food.
  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: The nonfood contact surface of a 3-shelf wheeled cart is not corrosion resistant, nonabsorbent, and/or smooth (bottom level observed to be rusted).
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or that requires frequent cleaning, must be smooth, durable, and nonabsorbent. The rusted surface should be painted. Alternatively, discard the equipment and replace with a cart that meets requirements for maintenance and cleanability.
  • Temperature Measuring Devices
    Observation: There were no temperature-measuring devices located in the interior of the "True" wheeled reach-in pass-through hot-boxes/coolers that are positioned between the kitchen and the serving line. (The temperature-measuring devices built into these units do have read-out displays on their exteriors.) Temperature-measuring devices could not be found for the self-service multi-shelved display cases (as for milk, etc.) positioned at the beginning of each serving line.
    Correction: Provide a temperature-measuring device within the interior of all hot or cold holding equipment used to store potentially hazardous foods, so that employees can routinely monitor the ambient air temperature inside the units. The sensors of these devices shall be positioned in the warmest part of cooling units, and the coolest part of warming units.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The dishwashing machine was observed in a state of disrepair: the booster heater was stated to be non-functional and awaiting service from manufacturer/installer.
    Correction: Repair the dishwashing machine to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections under the 3-vat sink are leaking: the drain from this sink debouches onto the floor grate that was installed as part of the air gap in such a way, that wastewater floods out onto the floor in front of the sink. NOTE: this condition, and various possible corrective measures, were discussed with construction/maintenance personnel and kitchen staff during this inspection
    Correction: however this condition was not noted on the original inspection form at the time of inspection. It is included now as a reminder.
  • Refuse - Storage Areas, Rooms, and Receptacles, Capacity and Availability
    Observation: There are no refuse containers in the areas immediately adjacent to the hand sinks.
    Correction: Provide a refuse container for the disposal of paper towels in areas immediately adjacent to all hand sinks.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Floor, by leg of fixed work table located outside the entrance of the walk-in cooler, is not smooth and easily cleanable: a tile at base of table leg is cracked and chipped.
    Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.
  • Toilet Rooms - Enclosed
    Observation: Toilet room door is not provided with a self- closing device.
    Correction: Provide toilet room door that is tight fitting and has self-closing device. Completely enclosed toilet facilities minimize the potential for the spread of disease by the movement of flies and other insects between the toilet facility and food preparation areas.
  • Outer Openings - Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents: a gap was observed across the bottom of the rear door when closed. Completion of weatherstripping of door awaits completion of paving of the parking lot and installation of an entry ramp.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
  • Handwashing Cleanser - Availability
    Observation: Soap was not provided at the hand washing lavatories in the facility (dispensers had not been filled).
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

A walk-through with school's construction staff and food-service supervisor prior to actual pre-opening inspection. Construction was nearly complete, except for minor details like paint touch-ups or finishing the window sill in customer area of serving line or installing soap and towel dispensers at hand sinks. All observations in substantial compliance (air gaps/backflow prevention devices where required
