The employees have knowledge about the employee health policy but could not show the signed documents. Please train all employees again and keep the signed agreements on file for review.

  • Hair Restraint Effectiveness (corrected on site)
    Observation: Employees observed working in the food service area without proper hair restraints. Observed employee wearing visor that was not restraining hair properly.
    Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff.
  • Equipment / Fixed / Space / Seal
    Observation: The following equipment is not installed in a manner that allows accessibility for cleaning, nor is it installed with closely abutting surfaces to minimize the need for cleaning: Gap between the 3-vat sink and wall on left side.
    Correction: Equipment that is fixed because it is not easily movable shall be installed so that it is: 1) spaced to allow access for cleaning along the sides, behind, and above the equipment, 2) spaced from adjoining equipment, walls, and ceilings a distance of not more than 1 millimeter or one thirty-second inch, and 3) sealed to adjoining equipment or walls if the equipment is exposed to spillage or seepage.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the permit to operate in a conspicuous location.
    Correction: A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. Note: The manager did not have CFM card.
    Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.
  • Handwashing Cleanser, Available at Handwashing Facilities
    Observation: Observed that hand soap was not provided at handwashing sink near the 3-vat sink area. It is broken.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.
  • Cleaning Frequency for Physical Facilities
    Observation: Observed that the drain under the 3-vat sink and area on the left side of 3-vat sink had black debris.
    Correction: Clean the physical facilities at a frequency necessary to ensure the protection and safe preparation of food.

  • Critical: Quaternary Ammonium Compound Sanitizer Solution Concentration per Use Directions (corrected on site)
    Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the SANITIZER SPRAY BOTTLE & 3 COMPARTMENT SINK with a concentration of <100 ppm total quaternary ammonium compound.
    Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.MANGER PLACED A CALL TO CONTRACTOR FOR REPAIR.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment.
  • Plumbing / Maintained in Good Repair
    Observation: REAR HAND SINK IS NOT WORKING AND FAUCETS ON 3 COMP. SINK DO NOT OPERATE CORRECTLY.
    Correction: A plumbing system shall be maintained in good repair.
  • Refuse Areas / Enclosures / Receptacles / Good Repair
    Observation: The waste storage container (DUMPSTER) is in disrepair. DUMPSTER HAS A GAP WIDE ENOUGH FOR RAT ENTRY AT BOTTOM (RUST) AND A HOLE IN FAR DOOR WIDE ENOUGH FOR RAT ENTRY.
    Correction: Storage areas, enclosures, and receptacles for refuse, recyclables, and returnables shall be maintained in good repair. REPLACE DUMPSER. (PREFERABLY WITH MORE RAT RESISTANT DUMPSTER WITH A MIN. 4 FOOT HIGH TOP LOADING ONLY DOORS, AND WITH A PLUG IN DRAIN HOLE.)
  • Toilet Tissue, Availability
    Observation: Observed that toilet tissue HOLDER was not provided at EMPLOYEE toilet.
    Correction: Provide toilet tissue HOLDER at each toilet to minimize hand contact with fecal waste.
  • Critical: Controlling Pests / Methods to Use When Pests Are Found
    Observation: Insufficiant methods are being used to monitor for pests (No one noticed). ABOUT 10 ACTIVE RAT TYPE HOLES TO THE REAR OF DUMPSTER COMPOUND AND OVER 100 RAT TYPE DROPPINGS NEAR DOORS OF BOTH DUMPSTERS.
    Correction: Pests are capable of transmitting disease to people. If pests are found on the premises methods shall be used to control their presence. HIRE A PROFESSIONAL AND WHEN ALL RAT TYPE BURROW HOLES REMAN FILLED FOR AT LEAST 1 WEEK, RATS HAVE BEEN EXTERMINATED OR GONE. ORAL INFO. PROVIDED.
  • Critical: Identifying Toxic Containers (corrected on site)
    Observation: Observed that containers of poisonous and toxic materials do not bear have legible manufacturer's labels. SANITIZER SPRAY BOTTLE ONLY SAYS CLEANER.
    Correction: All containers of poisonous and toxic materials shall bear a legible manufacturer's label to prevent accidental contamination of food and food contact surfaces.LABELED.
10/01/2014Routine
The purpose of this visit was to conduct a routine risk inspection. Water heater information: AO Smith DRE 52 920. Quaternary ammonium sanitizer in 3 vat sink: 300 ppm.
The food service establishment is not in compliance with having an Employee Health Policy. The Person-In-Charge is to provide training on the Employee Health Policy within 14 calendar days. Documentation (meeting attendance sheet/signed agreement form) is to be submitted to the Health Department within 14 days by fax or email that training has been completed.
NOTE: PLEASE REMEMBER TO PROVIDE SIGNED EMPLOYEE REPORTING AGREEMENTS TO HEALTH DEPT. WITHIN 14 DAYS. AS REQUIRED ABOVE.

  • Critical: Responsibility of PIC to Require Food Employees to Report GI Symptoms or Cuts, Disease, Medical Condition or Exposure to PIC
    Observation: Upon discussion with the person-in-charge, one or more of the elements of an effective employee health policy is either missing or incomplete. A complete employee health policy is required to be in place at the food establishment. At the time of this inspection, the Health Department provided and reviewed handouts and resource information.
    Correction: A complete employee health policy must have the following elements:
    1) Employee training on foodborne illness, particularly symptoms of illness and prevention of the Big Five illnesses (see "What is Foodborne Illness" handout)
  • Critical: Hands and Arms / Cleaning Procedure (corrected on site)
    Observation: Observed food employees using improper handwashing procedures. NOTE: Discussed with PIC.
    Correction: Food employees shall wash their hands and exposed portions of their arms by: 1) discarding used gloves
  • Critical: Handwashing / When to wash hands (corrected on site)
    Observation: A food employee failed to wash his or her hands before lifting prep table top lid covers (contaminating handles), after taking trash to the dumpster. NOTE: Discussed wiht PIC.
    Correction: ALL food employees shall wash their hands OFTEN especially after handling trash, using the restroom, handling raw meats, handling soiled equipment and utensils, BEFORE putting on clean gloves, etc. Proper handwashing frequencies will aid in the reduction of foodborne illness.
  • Warewash Equipment / Cleaning Frequency
    Observation: The compartment(s) of the following warewashing equipment are heavily soiled: Plastic vat used for washing screens.
    Correction: A warewashing machine, the compartments of sinks, basins, or other receptacles used for washing and rinsing equipment, utensils, or raw foods, or laundering wiping cloths and drainboards or other equipment used to substitute for drainboards shall be cleaned before use, throughout the day at a frequency necessary to prevent recontamination of equipment and utensils and to ensure that the equipment performs its intended function and if used at least every 24 hours.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink at the 3 vat sink being used to store scrub pads. NOTE: Discussed with PIC.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands.
