REMINDERS: 1) Check temperature of chicken and cheese in salad when it is delivered. If it is more than 41F please put in freezer to cool down to 41F or less. 2) Rotate the yogurt and salad at service line after every 2-3 lunches to make sure it stays under 41F. Observation: A strong offending odor is emanating from the refuse container stored outdoors.Observed some milk stains on ground, probably leak from the dumpster.
Correction: Soiled receptacles and waste handling units for refuse, recyclables, and returnables shall be cleaned at a frequency necessary to prevent them from developing a buildup of soil or becoming attractants for insects and rodents. Wash and/or replace the dumpster.

Observation: Observed the lack of an approved backflow prevention device on the water supply system. OVEN/STEAMER WITH WATER FILTER SYSTEM UPSTREAM HAS NO APPARENT BACK FLOW PREVENTION DEVICE.
Correction: A plumbing system shall be installed to preclude backflow of a solid, liquid, or gas contaminant into the water supply system at each point of use at the food establishment, including on a hose bib if a hose is attached or on a hose bib if a hose is not attached and backflow prevention is required by Law by installing an approved backflow prevention device as specified under 5-202.14. Observation: When tested, the quaternary ammonium compound-based sanitizing solution was observed to be at a concentration less than what is required for effective sanitization of food-contact surfaces of equipment and utensils. Observed the sanitizing solution in the 3 comp. sink & wash cloth buckets with a concentration of 0.0 ppm total quaternary ammonium compound.
Correction: The quaternary ammonium sanitizer used in this facility shall have a minimum concentration of at least 200 ppm OR a concentration between 150-400ppm based on the manufacturer's use directions and using the appropriate test kit.
- Critical: Common Name on Working Containers of Toxics (Cleaners, Sanitizers) (corrected on site)
Observation: Working containers of poisonous and toxic materials are not properly labeled with a common name. ONE TABLE SPRAY BOTTLE SANITIZER NOT LABELED AND 1 HARD TO READ & LABELED "ADULT ONLY".
Correction: Label all working containers of poisonous and toxic materials, including cleaners and sanitizers, with a common name to prevent the dangerous misuse of these materials.

Observation: The diameter of the air gap between the water supply inlet and the flood level rim of the 1.5 " pipe on the left drain of the 3 vat sink is less than 1 inch.;An air gap between the water supply inlet and the flood level rim of the plumbing fixture, equipment, or nonfood equipment shall be at least twice the diameter of the water supply inlet and may not be less than one inch (Above the lowest point of the rim).
