Food supplier are US Foods and Sam's Club. Chicken fillet and ground beef come raw frozen. Disucssed employee health, form 1B is used. Discussed cooling and reheating procedures. Discussed setting up 3 compartment sink. recommend cleaning can opener.
  • Critical: Warewashing - Manual & Mechanical Warewashing Equipment, Chemical Sanitization - Temperature, pH, Concentration & Hardness* (corrected on site)
    Observation: Quaternary Ammonium sanitizing solution used was not at an acceptable concentration. Observed sanitizer concentration from dispensing unit at 0ppm.
    Correction: Adjust the sanitizing solution to 150-400 parts per million or otherwise identified by manufacturer's recommendations. PIC will use sanitizer tablets until unit is fixed. Corrected to 200ppm.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: The following areas noted in need of cleaning: Under the 3 compartment sink, ubder along the grill line, under cold holding units.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Discussed employee health, reheating time/temperature for canned cheese which is hot held, handwashing and glove use. Employee health information posted. EHSS suggested reviewing information with staff. Burgers are not undercooked and no consumer advisory is needed. Chicken and hamburgers are cooked when ordered. Recommend cleaning and/or replacing ceiling tiles where needed.
  • Non-Food Contact Surfaces (repeated violation)
    Observation: The nonfood contact surface of the ice cream freezer has accumulations of grime and debris. Observed mold on the top of the ice cream freezer.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Observed the floor in need of cleaning in the following areas:
    - under the soda box stand
    - under the handsink
    - around the grease trap at the 3 basin sink
    - area between ice machine and door

    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food.     Observation: Observed commercial, precooked chicken nuggets cold holding at improper temperatures for unknown length of time.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.
  • Non-Food Contact Surfaces
    Observation: Observed mold around gaskets and top rim of ice cream freezers.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Observed floor tiles missing at the base of the True reach in cooler. The floor is not maintained in good repair
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
