4) Seal perimeter of platform at counter walls.
5) Clean refrigeration coils of coolers & freezer.
6) Replace damaged ceiling panels.
Facility approved for permit.

Facility not approved form permit.
Correct the following items:
1) Provide evidence of employee health policy.
2) Repair both prep coolers to maintain food at 41 degrees or less.
3) Replace dead ceiling light lamps.
4) Clean exhaust hood canopy.
5) Clean floor sink below prep sink.
6) Clean inside rear prep coolers.
7) Replace broken door gasket in rear prep cooler.
8) Clean flour below pizza oven.
9) Clean inside micro-wave oven.
10) Store flour / corn meal in sealed containers. Recommend mobile bins.
11) Varnish or paint bare particleboard platform at front counter.
12) Seal perimeter of platform at counter walls.
13) Clean refrigeration coils of prep coolers, drink cooler, & upright freezer.
14) Replace damaged ceiling panels.
