Hair Restraint Effectiveness Observation: Employees observed working in the food service area without proper hair restraints. Correction: Food employees shall wear hair restraints such as hats, visors, hair nets or beard nets to effectively keep their hair from contacting exposed food, clean equipment, utensils, linens, and unwrapped single-service items. This section does not apply to food employees such as counter staff who ONLY serve beverages and wrapped or packaged foods, hostesses, and wait staff. Food Protection from Contamination / Package, Cover, Wrap (repeated violation) Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: fruit in the freezer is not covered. Correction: Foods shall remain covered at all times. Critical: Separation of Toxics by Spacing or Partitioning (corrected on site) Observation: Observed that poisonous and toxic materials are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. A spray bottle of Windex was improperly stored with single service plates. The Windex was relocated during the inspection. This visit was made to conduct a complaint investigation. Spoke to manager who explained protein bars are offered at a 50% discount when expiration is within 90 days. For example if a protein bar is scheduled to expire in March they will offer the bar at a 50%discount in January. No protein bars are offered for retail sale if past expiration date. At time of investigation, facility did not have any protein bars offered at the 50% discount. Observed protein bars for sale were within their expiration date, none expired. Schedule Food Safety training Critical: Employee Health Policy Requirements Observation: Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: FPM has no knowledge of the policy Correction: Create and implement an EHP that: 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, Nontyphoidal Salmonella, E. coli, or Shigella) Food Protection from Contamination / Package, Cover, Wrap (corrected on site) Observation: Unwrapped or uncovered food in the following location where the food is subject to contamination: open bags of different fruits in the chest freezer. This visit was made to conduct a complaint investigation regarding no running water in the facility. I spoke with the manager, Chris Prevatte, who reported that a sewer line break caused them to have to turn off the water until it is repaired, which he anticipates will be within a few hours. The pool was already closed upon my arrival with signs and barricades to prevent patrons from entering. The smoothie kitchen was closed upon my request. The area in the women's locker room where the sewage spilled into the building had already been cleaned before my arrival, and the toilets in the locker rooms were blocked off with yellow tape so that patrons do not use them. Equipment is cleaned with a pre-prepared sanitizer solution, so equipment is still being sanitized. Mr. Prevatte reported that the pool operating company, Capitol Pool, will be out to re-open the pool and check the water quality once water is restored to the building. I called the complainant and reported my findings. No violation noted during this evaluation.	11/18/2013	Complaint
This visit was made to conduct a routine evaluation. Use chlorine (household bleach) at a concentration of 50-100 ppm to sanitize washed and rinsed dishes until you obtain another container of sanitizer solution for your pre-mix system at the 3 compartment sink. Critical: Food-Contact Surface Sanitized before Use after Cleaning (corrected on site) Observation: The food-contact surfaces of the following equipment were not observed sanitized: blender containers and utensils. (No sanitizer solution present for the pre-mix system for 3 compartment sink. PIC agreed to use chlorine sanitizer solution until they obtain another container.) Correction: In conjunction with Section 4-703.11, the food-contact surfaces of equipment and utensils shall be sanitized before use and after cleaning.	10/10/2013	Risk Factor Assessment
Complaint was received regarding the cleanliness of the fitness center equipment. Complaint is not food related. Spoke with the manager on duty and visited each floor of the facility. Center encompasses 3 floors and has 15 paper towel and disinfectant spray stations. Facility also has 5 hand sanitizer stations with 3 next to water fountains and 1 each in child play room and aerobics area. Insufficient staff to wipe down each piece of equipment after use but sanitizer is available and convenient for clients to use. Will email the complainant with results of the investigation.
