    Observation: Foods on prep line for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.
    Correction: Discard food that is not cooked and served or served if ready-to-eat within 4 hours from the point in time when the food is removed from temperature control. Clearly label or otherwise mark the food with a 4-hour cook-serve or serve by time.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Mechanically vented hood filters not in good repair or have gaps in between filters.
    Correction: Repair or replace hood filters and/or eliminate gaps between filters.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Observed accumulations of dust, dirt, food residue or other debris on the following nonfood-food contact surfaces: ice machine.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose
    Observation: The handwash station at the back of the kitchen area is being used for purposes other than washing hands.
    Correction: The handwash facility identified above is to be used for washing hands only
  • Lighting, Intensity
    Observation: Less than 10 foot candles of light was noted in the dry storage area.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
  • Dressing Rooms - Using Dressing Rooms and Lockers
    Observation: Employees are not using the dressing rooms, designated area or lockers provided.
    Correction: Employees that regularly change their clothes in the establishment should use the dressing room or lockers provided.

    Correction: Ensure employees are not wearing jewelry on their arms and hands while preparing food. This section does not apply to a plain ring such as a wedding band.
  • Food Storage - Clean and Dry Location
    Observation: Food stored on the floor or food stored less than 6" above the floor.;Elevate food storage onto approved shelving with minimum 6" legs or casters.
  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located in the low boy units.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Pests - Controlling Pests*
    Observation: Harborage conditions exist
    Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

I went to Wendy's on 3/17/18. I arrived around 10 pm in the drive thru with 2 cars in front of me. It didn't take long to place my order, however it did take a while for me to pay and received my order. I ordered a number one with a large sweet tea and also a JBC (as the cashier called it). My total was $10 and some change. When I received my receipt after payment I noticed the time stamp on the receipt said 10:07 pm but I just shrugged it off and placed the receipt in my bag and proceeded to work. I got to work and opened my hamburger and it was literally 2 pieces a meat with a squirt of mayo and ketchup. The wrapping on the Single was disheveled. The burger looked like it had been sitting for a minute and frankly it looked disgusting. I opened the JBC and it was ok. Neither burger was hot and the JBC looked fresher than single, but I ended up eating only the fries and drinking the tea. This meal was my dinner for the night due to the fact that I work overnight. I went to Wendy's because the night before it took me 20 minutes to get my food at McDonalds, which is directly across the street. I think I will just take my chances with McDonald's next time.
