    Observation: The employee observed eating chicken in food prep areas where they may contaminate food, clean equipment, utensils or other items needing protection.
    Correction: Provide a designated area where employees may eat so as not to contaminate exposed food, clean equipment, utensils, and linens, or unwrapped single-service and single-use articles, or other items needing protection.
  • Utensils - In-Use - Between-Use Storage (corrected on site) (repeated violation)
    Observation: In-use utensils improperly stored between use in that ice scoop observed store on ice with handle touching the ice.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

Complaint was received stating that roach was found in a cup, maybe from ice machine. Manager on duty was aware of complaint. During the inspection no live roaches were observed. Three dead roaches were observed on storage rack in rear dishwashing area. Standing water and old coffee beans were observed on floor under coffee/drinks prep station. Standing water observed on floor in rear dry storage and dishwashing area. Cracks and gaps observed on and between tiles. Must be repaired. Ice machine observed clean. According to the manager the last pest control treatment was on 9/28/15. Continue pest control treatment to the facility and maintain the facility clean and without pest.
No violation noted during this evaluation.

    Observation: Cooked eggs, ham and milk refrigerated at 45-46 degrees F in walk in refrigerator. Said unit registered at 45 degrees F.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Unit temperature must be lowered to 41 degrees F or less.

Note:
During the inspection the operator kept the door closed of the walk in refrigerator and the temperature of said unit was lowering. Registered at 43 degree F. Sai unit must be lowered to 41 degrees F or less to hold potentially hazardous food.

    Observation: The ice used for customer consumption is contaminated with water dripping from inner panel of said ice machine with heavy accumulation of yellow mildew.

A complain was received stating cooked roach was in order of hash brown of the customer. During the inspection two live and five dead roaches were observed in beverages/coffee prep area also harborage conditions were observed throughout the establishment (refer to observed violations mentioned above for detail). Operator was instructed to work with pest control company more frequently to minimize the the presence of pest on premises and also eliminate harborage conditions and remove dead and trapped pests.

    Observation: Two live roaches observed on glue traps in coffee/beverages prep area.
    Correction: Pest must be controlled to minimize their presence on the premises by (A) routinely inspecting incoming shipments of the food and supplies (B) routinely inspecting the premises for evidence of pests (C) using pest control methods such as trapping devices (D) eliminating harborage conditions.
  • Pests - Controlling Pests*
    Observation: Harborage conditions exist in that floor in the kitchen, food prep area, dry goods storage area and dry food storage area observed with accumulation of water, grease and old food particles.
    Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.

    Observation: Dead or trapped five dead and two live roaches were found in the control device (glue trap).

  • Physical Facilities in Good Repair
    Observation: Condensation/plumbing leak @ soda lines behind the ice cream machines is draining into an overflowing trash can. Several stacks of cups have fallen to floor behing the machines.
    Correction: Repair cauce of leak. Discard the cups. (Cups have been discarded)
