    Observation: The nonfood contact surface of the smoothie container/Mixer cart had accumulations of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Food Debris and grime on conveyor belt and interior surfaces of oven.
    Correction: Clean and sanitize the food-contact surface, reapply the lubricant in a manner that does not introduce contaminants to food contact surfaces.

    Observation: The nonfood contact surface of the Middle door gasket to the Victory Prep Line Area Reach-in Standup cooler is not designed or constructed to be easily cleanable.(Torn Gasket on door).Repeat violation.
    Correction: Alter or replace this item to provide a surface that is free of unnecessary ledges, projections, and crevices.
  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located in the Prep line lowboy coolers.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.Correct immediately.
  • Temperature - Food Temperature Measuring Devices - Provided
    Observation: The certified food manager could not provide a food temperature measuring device.
    Correction: Obtain at least one food temperature measuring device so employees can properly monitor the temperature of food items in the establishment.Correct immediately.
  • Sanitizing Solutions, Testing Devices
    Observation: There was no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a QAC test kit so that employees can accurately monitor the concentration of the sanitizing solutions and maintain them at a safe and effective level.

    Observation: Smoothie Station Lowboy refrigerator was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Leaking condensate water to floor area).
    Correction: Repair the unit to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the cooler, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: Observed accumulations of grime and debris on the following food contact surfaces: Smoothie station interior bottom shelve of lowboy refrigerator and roller wire on top level of rolling oven under hood system
    Correction: Clean and sanitize these surfaces for food contact.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: The automatic handwashing facility located at prep area stations is not approved in accordance with manufacturer specifications.(No water flow,Sink is in disrepai).
    Correction: Reconnect the automatic handwashing sink to provide for effective handwashing and in accordance with manufacturer specifications.(Running hot and cold water).

    Observation: Floor and wall juncture in Smoothie station floor/counter junction is not sealed.(Wool pad secured in wooden hole area.
    Correction: Seal floor and counterl juncture in rooms where cleaning methods are used.
  • Hand Drying Provision (repeated violation)
    Observation: No disposable towels were provided a the hand washing lavatory in the kitchen handsink.(Next to office area)
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials
  • Critical: Pests - Controlling Pests*
    Observation: Methods are not being used to control pests. (Fruit Flies)
    Correction: When pests are found use methods to such as trapping devices or other means of pest control. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces.Present activity of flies.Have Pest company treat for these pest.Have copy of invoice for treatment within 2 weeks from today.

    Observation: Facility is storing unnecessary items to the operation or maintenance of the establishment.Boxes.
    Correction: Remove all unnecessary items from the facility. The presence of unnecessary articles, including equipment which is no longer used, makes regular and effective cleaning more difficult and less likely. It can also provide harborage for insects and rodents.

    Observation: Handwashing facilities are not maintained. Hot water handle/water not operatering correctly.

If unable to repair the ddor gasket, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Unlabeled food containers.( Various Squeeze bottles not labeled and incorrectly labeled).Discussed with CFM about this issued and she corrected on site).
    Correction: Label working containers with the common name of its contents. Food containers shall be labeled in a language that is understood by the regulatory authority and by the employee. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.Mgt/CFM to have meeting with employees about this issue.

    Observation: Observed accumulations of grime and debris on the following food contact surfaces: Interior bottom shelve area of Beverage Air lowboy refrigerator at Smoothie Station.

    Correction: Refinish or replace the floor material at the area designated so it is: 1. smooth, durable and easily cleanable 2.nonabsorbent.(free of cracks and chips).
  • Light Bulbs Protective Shielding
    Observation: (Light Bulbs) in Dry storage overhead not protected against breakage by a shield.
    Correction: Protect lamp against breakage by a shield surrounding and extending beyond the bulb so only the face of the bulb is exposed/or complete shield installed/missing shield.

    Observation: Smoothie Station lowboy refrigerator had pooling of condensate water in bottom shelve area which was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.
    Correction: Repair the unit so no pooling of condensate water forms in bottom shelve area which will restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the unit, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Water from the handwashing sink at Back of kitchen next to the Victory refrigerator was measured at a temperature less than 100°F and water pressure was not sufficient.
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens and increse water pressure flow.
  • Lighting, Intensity
    Observation: Less than 50 foot candles of light was noted in the prep area in kitchen/Oven area.(One light out)
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor
