Discussed the following with the person in charge (PIC):
1. Food sources are Sam's Club & Walmart.
2. Fire extinguishers were inspected in December 2014.
3. No eggs are served at this facility. Raw shell eggs are used for baking only.
4. All foods are purchased precooked except for the raw shell eggs and ground beef.

  • Equipment - Good Repair and Proper Adjustment
    Observation: The interior of the infant's room reach-in refrigerator was observed in a condition that prevents necessary maintenance (frost buildup). This may be affecting the proper functioning of the unit as the ambient air temperature in the unit was observed at 44°F,
    Correction: Repair or replace the reach-in refrigerator to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The (3) door gaskets to the True reach-in cooler are in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.
10/14/2015Routine
Discussed the following with the PIC:
1. The (PIC) informed the EHSS upon arrival that at about 11:00 A.M. she noticed that the foods in the reach-in freezer had thawed and that the temperature of the freezer went up. She then realized that the circuit breaker had tripped and turned off the power supply to the freezer. Upon my arrival, the freezer was turned back on. The EHSS took temperatures of the TCS foods stored in the freezer and the temperatures of all of the foods were below 41°F (between 31-35°F). The PIC relocated the chicken nuggets and sausage patties to the reach-in cooler to be reheated and served tomorrow during breakfast and lunch. The PIC discarded the "dinosaur" chicken nuggets. The remaining foods (broccoli, fish sticks, & pizza rolls) will be refrozen.
2. Food source is Sam's Club.
3. Observed the employee health policy (FDA Form 1-B) signed by all staff and kept on file in the office.
4. No eggs are served at this facility. Raw shell eggs are used for baking only.
5. All foods are purchased precooked except for the raw shell eggs and ground beef.
6. Ensure that gloves are changed between tasks.

  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Almond milk cold holding at improper temperatures in the infant's room reach-in cooler. The temperature of the milk was 47°F. The director removed the milk and formulas from the unit and placed them into the 3 door reach-in cooler.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria. Do not store temperature control for safety (TCS) foods in the infant's room refrigerator until it is capable of holding the food at or below 41°F.

Discussed the following with the person in charge (PIC):
1. Recommend having a written employee health policy. FDA Form 1-B was given to the PIC.
2. The facility does not serve eggs. The shell eggs are used for baking only.
3. All meat and poultry is purchased precooked except for the ground beef and the eggs used for baking.
Observed the overall facility clean.

  • Temperature - Food Temperature Measuring Devices - Provided
    Observation: The Person in Charge could not produce an appropriate food temperature measuring device to verify cooking temperatures of thin foods such as hamburgers.
    Correction: A temperature measuring device with a suitable small-diameter probe that is designed to measure the temperature of thin masses shall be provided and readily accessible to accurately measure the temperature in thin foods such as meat patties and fish filets.
  • Equipment - Cooking and Baking Equipment (corrected on site)
    Observation: The cavity of the microwave oven is observed soiled.
    Correction: Using the manufacturer's recommended cleaning procedure, clean the microwave oven at least every 24 hours.
  • Non-Food Contact Surfaces (corrected on site)
    Observation: The nonfood contact surface of the floor fan has accumulations of dust.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of dust.
  • Plumbing System Maintained in Good Repair
    Observation: The handwashing sink in the kitchen is slow to drain.
    Correction: Plumbing systems and components shall be maintained in good repair.

Observed lighting in kitchen food preparation area at 10 foot-candles intensity. Two (2) light florescent bulbs were replaced in unit for kitchen. 40 foot candles and greater intensity available. Verified that all staff take employee heath review as a part of their orientation. Form 1B was signed by staff.

    Observation: Observed sanitizer solution in classroom above 200 ppm chlorine concentration for cleaning of lunch/snack tables.
    Correction: Recommend all classrooms adjust and use spray sanitizer level of chlorine to 50-200 ppm. Food service tables should be cleaned before and after food service and consumption.
  • Pests - Controlling Pests*
    Observation: Observed flies in areas of facility.
    Correction: Recommend safe, food grade fly catcher be used to prevent infestation.

Observed broken thermometer in RIC True. Very clean, well organized. Cooking raw frozen hamburger and chicken. Establishment should moderate priority.
  • Cloths - Wiping Cloths - Use Limitation
    Observation: Observed sanitizer container at strength of less than 50 ppm chlorine. Not available to be used for cloths to clean and sanitize counter and table surfaces.
    Correction: Recommend sanitizer solution be available when counters and tables are cleaned and sanitized. CORRECTED.
  • Critical: Food - Potentially Hazardous Food - Cold Holding*
    Observation: Observed hot dogsat 43 degrees cold holding
    Correction: Recommend hot dogs be moved to RIF to attain temperature of 41 degrees or less.

Observed sanitizer level too high (+300 ppm) in sanitizer container. Recommend adjust chlorine level to 200 ppm or lower for spray sanitizer. Container should be labeled as to contents. Form 1B Employee Health reviewed and signed by all employees. Betty Hayes Director.
