| The unit was not in operation at the time of inspection. The service area remains the same (The Caboose). The EHSS will send the person in charge a sample employee health policy (FDA Form 1-B). Recommend reviewing the policy with the staff that will be working the cart and having them sign a copy of the policy to keep on file.

| Cart will operate occasionally on Saturdays at the Ashland train station, Ashland 1st Thursday events, at the Ashland Street Parties (3x during summer), Bluemont Concert series on Sundays in August. Service area to be determined and EHSS provided service letter for completion and return. Cart will be stored at the Ashland Main Street Office. Freezer will remain plugged in as needed. Ice cream is purchased through and delivered to Cross Brothers grocery store across the street from the Ashland Main Street Office and stored when needed. Ice cream cones will be purchased the day of the event from Restaurant Depot and placed on cart. Single service items -- spoons, cups, napkins, gloves are stored on cart in covered plastic bin. Hand sanitizer, soap, paper towels to be stored on cart in plastic bin. Disposable ice cream scoops to be used. Handwashing station is a temporary-type set up due to no hand contact with ice cream and minimal service operation,per regulations. Only two flavors of ice cream are served. No toppings offered.
