  • Hair Restraints - Effectiveness (repeated violation)
    Observation: One of the employees was observed working in the food service area without proper hair restraints.
    Correction: All employees, while working behind the counter, and in other kitchen areas, are required to wear a hair covering such as a hat, a hair net, an open visor, a scarf, or some other type of hair covering that we have approved.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: There area of the wall, surrounding the second kitchen handwashing sink, in the small room, where the three-compartment (dishwashing) sink is located, has rough surfaces where the finish is peeling off, exposing surfaces which are not cleanable, at the bacterial level. Also, the area of the wall, on the "dirty dishes drain-board side," of the three-compartment (dishwashing) sink, has rough surfaces where the finish is peeling off, exposing surfaces which are not cleanable at the bacterial level.
    Correction: We require that all reas of the kitchen walls (and floors, and ceilings) be maintained so that they are easily cleanable at all times. These areas of the kitchen walls need to be repaired, as soon as possible.

    Observation: The wall to the left of the handwashing sink, in the room where the three compartment (dishwashing) sink is located, has rough surfaces. These wall surfaces are no longer easily cleanable.
    Correction: We require that all walls (and floors, and ceilings) be maintained in good repair. The wall repairs next to the handwashing sink, in the room where the three compartment sink is located, need to be made smooth, and repainted, prior to renewal of the permit, on, or before 31-Dec-2015.

    Observation: There is no "self-closing mechanism" on the entrance door to the employee restroom.
    Correction: We require that there be a "self closing mechanism" on the entrance doors to all restrooms. A "self-closer" needs to be installed on the employee restroom entrance door..

  • Equipment - Good Repair and Proper Adjustment
    Observation: In the white chest freezer (located near the entrance to the walk-in refrigerator), the plastic surface inside the top of the freezer has come loose, and the gasket inside the top of the freezer is torn-rough surfaces are exposed-not cleanable at the bacterial level.
    Correction: We require that all areas inside the freezer be smooth and easily cleanable at all times. The top to this chest freezer needs to be replaced, or repaired, so that all surfaces are easily cleanable.
