Observation: There was no temperature measuring device located in the refrigerator unit.
Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

Investigation of complaint shows that water is being drained from a hole in the ceiling in the kitchen through a tube. The tube empties into buckets on the floor. The hole is over the dish sink and food prep area and poses a risk for contamination. Advised operator to cease food and dishwashing operations until the ceiling has been repaired and no longer drains water. Operator is to contact PHD when repairs are made.
