    Correction: Remove any unused or non-functioning equipment from the premises.

    Observation: The following nonfood contact surfaces have accumulations of grime and debris: outside of the robot coupe, outside of the blender stand, inside of the fryer cabinets, all of the storage shelves, and the sprayer nozzle.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food.     Observation: A food employee failed to wash his hands before engaging in food preparation after making a delivery and before putting on a new pair of single service gloves.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Per discussion with the food employee, she uses a bucket of water to clean her hands. Employee instructed by EHS to use the hand sinks in the kitchen.
    Correction: Instruct food employees to clean their hands in a handwashing lavatory. Food employees may not clean their hands in a sink used for food preparation or utensil washing.

    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Observed single service and bulk food containers that were unlabeled near the walk in cooler.
    Correction: Label working containers with the common name of its contents. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.

    Observation: Observed unpackaged calamari stored in direct contact with undrained ice in the walk in cooler.
    Correction: Store unpackaged food in drained ice.

    Observation: Per conversation with the person in charge, all continuous use items are cleaned every 2 days.
    Correction: Ensure that all equipment that is in continual use, be washed, rinsed, and sanitized every 4 hours. Use only cleaned and sanitized utensils or equipment during food preparation.

    Observation: Observed rice scoops improperly stored in water that measured 78'F between uses.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Observed a large supply of wet wiping cloths improperly stored on the counter tops between use.
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: Observed food stored on the floor in the walk in cooler and under a table near the large prep unit. Observed uncovered foods in the walk in cooler and sandwich prep unit.
    Correction: Store food where it is not exposed to splash or install an approved durable, and cleanable barrier between the splash source and the food to prevent contamination.

    Observation: The chicken/vegetable patty was observed reheated to 99'F and observed peanut sauce that was sitting on the storage shelf at 62'F. Manager was instructed to reheat both items however failed to do so.
    Correction: Rapidly reheat food for hot holding to 165°F or above within 2 hours.
  • Critical: Cooling*
    Observation: Observed the dumplings sitting on a storage rack at 72'F and the tofu at 64'F on the sandwich prep unit were noted not being adequately cooled to prevent the growth of harmful bacteria.
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses.

    Observation: Observed bean sprouts - 53'F on top of the sandwich prep unit, Thai noodles on the prep table - 48'F, sweet potato/curry/onion mix - 51'F on the table near the rear door - all cold holding at improper temperatures
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: All of the prepared ready-to-eat (RTE) foods like the sauces, rice patties, and noodles in the refrigeration units are not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: Observed garlic/oil mixture for which time rather than temperature is being used as a control was not labeled or otherwise marked with a 4 hour cook, serve or discard by time.

    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent.

    Correction: Ensure all employees wear hair restraints such as hats, hair coverings or nets, beard restraints, and clothing that covers body hair, that are designed and worn to effectively keep their hair from contacting exposed food, clean equipment, utensils and linens, and unwrapped single-service and single-use articles.     Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: The prepared ready-to-eat (RTE) sauces and rice in the walk in cooler are not properly dated for disposition.

If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The establishment is currently using an ice-bath to try to keep foods cold on the cook's line which is not sufficient to meet the food storage demands.
    Correction: Provide an additional refrigerator necessary to maintain food items at 41'F or below. Improper food storage temperatures are a major contributing factor to foodborne illness.

    Observation: The person in charge could not provide a small probe food temperature measuring device.

    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.

Observed that all of the garbage from days before this inspection, along with used paint cans, are stored at the rear of the restaurant. Discarded products with food residue are being stored outside of refuse containers, outside of the establishment.
    Correction: Garbage must be taken to dumpster that are provided. Provide enough pest and rodent proof refuse containers for the storage of all waste with food residue.

    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
