    Observation: Milk at 45*F cold holding at improper temperatures in milk line display #1.
    Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Time will be used until unit is fixed, all extra milk in this unit is to be discarded after lunch. Milk was discarded during inspection.
  • Equipment - Good Repair and Proper Adjustment (repeated violation)
    Observation: The following equipment was observed in a state of disrepair and damaged: can opener blade outer coating worn off, and the racks of the 4 display refrigerators on the front line and walk-in cooler rusting.
    Correction: Repair the to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.

    Observation: The following equipment was observed in a state of disrepair and damaged: shelves in counter top refrigerators on the serving line are rusting.
    Correction: Repair to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
  • Toilet Room Receptacle Covered
    Observation: There is no covered refuse container for the disposal of feminine napkins in the employee restroom.
    Correction: Install a covered refuse container for the disposal of feminine napkins in the ladies room stall.
  • Pests - Controlling Pests*
    Observation: Roaches seen in kitchen. 5 roaches were seen alive in the utensil storage drawer, on a prep table and in the cardboard box of aluminum foil dispenser. Dead raoches were found in the ice cream freezer.
    Correction: Eliminate harborage conditions. Insects and other pests are capable of transmitting disease to man by contaminating food and food-contact surfaces. Routine pest management is being provided every two weeks to treat for roaches. The facility is being maintained very clean.

    Observation: The condensor line is slowly leaking in both walk-in freezers. Ice is accumulating below the lines.
    Correction: Repair and maintain all plumbing components and fixtures.

    Observation: Condenser line in walk-in freezer is slowly leaking and ice is built up on pipe and on surface below.
    Correction: Repair and maintain all plumbing components and fixtures.

  • Nonfood-Contact Surfaces - Corrosion Resistant/Nonabsorbent
    Observation: Bare wood shelving used in the mop room area to store chemicals.
    Correction: Paint or seal wood that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent. Discontinue the use of bare wood in the facility.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The table mounted can opener and vegetable chopper blade was rusted.
    Correction: Disassemble the can opener and vegetable chopper, clean and sanitize the assembly, remove and replace the blade. Can opener blades must be kept sharp to prevent creation of metal fragments that can contaminate food in the can when opened.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Shelving in the walk-in cooler had accumulation of mold and rust which prevents necessary maintenance and cleaning.
    Correction: Clean or replace the shelving to maintain cleanability.
  • Non-Food Contact Surfaces
    Observation: Multiple knives had debris and soil accumulation on the handles.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted.
  • Physical Facilities in Good Repair
    Observation: The following areas are in need of repair.
    1. Floors under the steam kettle were stained with rust.
    2. Paint on the mop room floor was deteriorating (painted).

    Correction: 1.Repair or replace tiles to remove rust. Poor repair and maintenance compromises the functionality of the physical facilities.
    2. Repaint surface of the mop room floor.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: The wall located in the dry storage room above the walk-in cooler was noted in need of cleaning. Dust from the ventilation fan had blown accumulations of dust onto the walls.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

The single-door display reach in on the left serve line is not properly cold holding and is not being used to store potentially hazardous foods (PHF's), unit will need to be repaired and properly cold holding at 41*F or lower if it is going to be used again to store PHF's. In the event of a change in operator or remodeling, the facility must be brought up to the current code requirements.     Observation: Shelving in both display refrigerators on serve line and walk-in refrigerator is rusting and is in a condition that prevents necessary maintenance and easy cleaning.

    Observation: Plumbing in both walk-in freezers observed leaking by the condenser. An insulated pipe in the dry goods storage area is no longer wrapped due to deterioration of the material used to wrap it and insulation is exposed.
    Correction: Repair and maintain all plumbing components and fixtures. Pipe should be re-wrapped so insulation is no longer exposed.
  • Floors, Walls, and Ceilings - Cleanability
    Observation: Floor and tile grout below steam kettle observed corroding and is no longer smooth and easily cleanable.
    Correction: Repair or replace floor or floor covering to make it smooth and easily cleanable.
  • Lighting, Intensity
    Observation: Less than 10 foot candles of light was noted in the storeroom walk-in freezer.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
