Food suppliers are US Foods and Flowers Bakery. Observed well organized facility. Discussed the following with the PIC: 1. Employee health, reviewed eLearning and observed Form 1B. 2. Recommend placing thermometer in warmest part of cold holding units. 3. Recommend replacing fly tape. 4. Discussed cooling and reheating procedures for chili. 5. Ensure handwashing is only done at designated handsinks and between change of gloves.
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Shredded cheese (49°) cold holding at improper temperatures on prep line. EHSS observed container of cheese stored on top of ice.
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. Per the PIC, cheese was on counter less than 1 hour.
Equipment - Fixed Equipment, Spacing or Sealing (repeated violation) Observation: The handsink located in the front of the kitchen is not sealed to adjoining wall.
Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.
Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils Observation: Observed accumulations of encrusted grease deposits or other soil on the following food contact surfaces: Interior of the Ice Machine
Correction: Clean and sanitize these surfaces for food contact.

Food suppliers are US Foods and Flower Baking Company. Ground Beef, Breaded Chicken, and Sausage are received raw frozen. Fire Suppression system was serviced in April 2015. LPCO is Orkin. Observed good handwashing and glove use. EHS discussed the following with the manager: 1. Employee Health
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: Sliced American Cheese cold holding at improper temperatures
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. PIC stated cheese had been in unit greater than 4 hours. PIC discarded cheese.

Risk Factor
Discussed the following with the person in charge (PIC): 1. Provide a new door gasket for one door on the sandwich prep unit. The door gasket is currently broken. 2. Ensure your employee health policy is reviewed with the staff frequently. Recommend posting the reportable symptoms and illnesses in an area where staff can review it frequently. 3. Do not use the milkshake machine until it is capable of holding the mix at or below 41°F. Clean and sanitize the machine prior to putting it back in use. Observed good hot holding temperatures of foods.
Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site) Observation: The milkshake mix in the hopper is cold holding at improper temperatures. The temperature of the mix is 49°F. The PIC has put the machine out of service until it is repaired and then cleaned and sanitized. The mix will be discarded.
Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
Equipment - Fixed Equipment, Spacing or Sealing Observation: The hand washing sinks in both restrooms and the handwashing sink located at the front entrance to the kitchen is not sealed to adjoining equipment or walls.
Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.
Non-Food Contact Surfaces Observation: The nonfood contact surfaces of the following equipment has accumulations of grime and debris:
1. Interior of the sandwich prep unit,
2. Shelf above the steamer on the prep line,
3. Exterior of the seasoning containers on the prep line,
4. Condenser fan covers in the walk-in cooler (dust buildup).
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
Handwashing Lavatory, Water Temperature, and Flow Observation: Water from the handwashing sink in the men's restroom was measured at a temperature less than 100°F.
Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.
