    Observation: The person in charge failed to state the required temperatures and holding times for the following conditions of potentially hazardous food: (1) Hot holding, (2) Cooling, and (3) Reheating. Handouts were left with the PIC.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.
  • Critical: Hands and Arms Cleaning Procedure* (corrected on site)
    Observation: Improper handwashing procedures observed. Observed employee rinse hands with plain water. Person in Charge discussed this with the employee.
    Correction: Ensure all food employees clean their hands and exposed portions of their arms with a cleaning compound in a lavatory by vigorously rubbing together the surfaces of their lathered hands and warms for at least 20 seconds and thoroughly rinsing with clean water.

    Observation: A food employee was observed rinsing her hands at the mop sink and then again at the three compartment sink.
    Correction: Instruct food employees to clean their hands in a handwashing lavatory. Food employees may not clean their hands in a sink used for food preparation or utensil washing.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation*
    Observation: Raw foods of animal origin stored over ready-to-eat (RTE) food in the refrigeration unit. Observed raw fish stored over cooked chicken and mashed plaintains in the reach in cooler.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Utensils - In-Use - Between-Use Storage (corrected on site)
    Observation: Dispensing utensils improperly stored between uses. Observed ice scoop handle stored in direct contact with the ice.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.
  • Critical: Reheating for Hot Holding* (corrected on site)
    Observation: The beef ribs were not reheated to a sufficient temperature and time to eliminate pathogenic bacteria. Observed beef ribs on the steam table at 121'F. Person in Charge instructed employee to place the ribs back on the flattop grill to continue reheating.
    Correction: Rapidly reheat food for hot holding to 165°F or above within 2 hours.
  • Critical: Cooling* (corrected on site)
    Observation: Beef soup noted not being adequately cooled to prevent the growth of harmful bacteria. Observed beef soup, cooked the night before, holding at 70'F. Person in Charge indicated the employees left the soup out at room temperature to begin cooling overnight. Person in Charge instructed employees to discard the soup. Also observed bone-in chicken that was cooked the previous night to be holding at 52'F in the reach in cooler. Person in Charge instructed employees to discard the chicken.
    Correction: Cool potentially hazardous foods from 135°F to 70°F within 2 hours and within a total of 6 hours from 135°F to 41°F. A longer time for cooling allows an ideal situation for bacterial growth. This has been shown to be the major contributing factor in many foodborne illnesses.
  • Cooling Methods
    Observation: The methods used for cooling were not adequate. Observed chicken placed in a large, deep and covered container. Soup pot was left out at room temperature overnight.
    Correction: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Pork and cheese (50'F), half and half (47'F) and shell eggs (60'F st) cold holding at improper temperatures in the food prep cooler. Foods were relocated.
    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.
  • Temperature Measuring Devices
    Observation: There was no temperature measuring device located in the prep cooler.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils* (corrected on site)
    Observation: The following utensils were observed soiled to sight and touch: 1) several pairs of tongs 2) spatula. PIC placed all the dirty utensils in the 3 compartment sink to be washed.
    Correction: Clean and sanitize these surfaces for food contact.
  • Non-Food Contact Surfaces
    Observation: The nonfood contact surface of the interior of the three door reach in freezer has an accumulation of grime and debris.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Equipment and Utensils, Air-Drying Required
    Observation: Food storage containers were found stacked while wet after cleaning and chemical sanitization.
    Correction: Items must be allowed to drain and air-dry before being stacked or stored to allow evaporation of chemical sanitizer and moisture that may encourage microbial growth.
  • Plumbing System Maintained in Good Repair
    Observation: Faucet at the three compartment sink in poor repair. Observed water leaking from the swan-neck faucet base.
    Correction: Repair and maintain all plumbing components and fixtures.
  • Critical: Toxics - Storage of Toxic Containers* (corrected on site)
    Observation: Container of ant and roach spray was observed stored among cleaning chemicals on the shelf above the three compartment sink.
    Correction: Store detergents, sanitizers, related cleaning or drying agents and caustics, acids, polishes and other chemicals separately from insecticides and rodenticides.

Discussed the following items with the Person in Charge:
1. Cleaning and repairs needed in physical facility.
2. Proper procedures for cleaning and sanitizing dishes and equipment. A handout for the proper set up of the three compartment sink was left with the Person in Charge.

  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site)
    Observation: The prepared ready-to-eat (RTE) foods, such as cooked chicken and various soups in the refrigeration units are not properly dated for disposition. PIC instructed staff to mark prepared food items with preparation dates.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Critical: Equipment and Utensils - Before Use After Cleaning*
    Observation: The food-contact surfaces of the equipment are not being sanitized. Per conversation with the Person in Charge, dishes are being pre-washed, then washed in hot soapy water with bleach. During inspection, a measurable amount of bleach was not detected in the water.
    Correction: After cleaning and rinsing of the food-contact surface, the surface shall be effectively sanitized before coming in contact with food and before use.

    Observation: Spanish Rice hot holding at improper temperature (100'F) Near the serving line. Rice was placed in the microwave and re-heated to 165'F and placed in the steam table.

    Observation: The food-contact surface of the cutting board on the prep unit was observed soiled to sight and touch. Cutting board was taken to the 3-compartment sink to be cleaned.
    Correction: Clean and sanitize these surfaces for food contact.

Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.
  • Ventilation Systems - Cleaning Ventilation Systems - Nuisance and Discharge Prohibition
    Observation: Intake and exhaust air ducts are not being cleaned. The intake ducts in the kitchen ceiling are dirty.

    Observation: Dispensing utensils improperly stored between uses. Observed several serving spoons stored in standing water next to the steam table. Water temperature was measured at 120'F. PIC discussed observation with staff, and they will begin storing the water in the steam table to ensure water holds at 135'F or higher.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Surfaces of the interior of the ice machine observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of the interior of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: The nonfood contact surface of the door gaskets to the prep cooler and the bottom shelf of the reach in freezer were observed with an accumulation of grime and debris.
