This visit was made to conduct a change of ownership inspection. Food establishment installed the following new hot water heater: State Sandblaster (SBD100250NIA 118) Input 250,000 BTU *New hot water heater is sufficient to meet the required recovery rate for the entire grocery store. *Approved for change of ownership, recommend issuance of Alexandria Health Department permit. This visit was made to conduct a change of ownership inspection. Prior to merge (re-opening), the following items require correction: 1. The waste-line for the sanitizing compartment has been altered to a larger waste-pipe. A significant amount of waste water is backing up onto the surrounding floors when grey water from sink basin is released. When all 3-compartments are released at the same time, grey water floods surrounding floor. Consultation with a plumber is necessary to evaluate waste water disposal at the 3-compartment sink to prevent the discharge onto floor surfaces. Proper control measures must be installed to prevent the backing up of sewage onto floor surfaces while maintaining indirect drainage. 2. Waste-line for the walk-in refrigerator currently discharges onto the floor adjacent to the floor drain and shall be properly plumbed with an indirect drainage. 3. Traulsen 2-door undercounter refrigerator is no longer operational and shall be removed and replace with another 2-door undercounter refrigerator for the cold holding storage of short term food items. 4. A barrier constructed of a smooth and easily cleanable surface shall be installed between the hand sink and the food prep table. 5. The current walk-in refrigerator is not being utilized to maximize cold holding. Walk-in refrigerator has two "dead spaces" due to inadequate shelf styles. Remove current shelving with properly aligned shelves to create additional cold holding space. 6. Ceiling tiles, walls, and floors throughout the kitchen have dust and grease accumulation and shall be cleaned and maintained. 7. Certified Food Protection Manager is not aware of filter replacement schedule for turbo air hood-less oven. Call maintenance company to discuss day-to-day requirements for sufficient operation and scheduling for filter replacement. 8. Walk-in refrigerator floor has an accumulation of dust and debri and shall be cleaned and maintained. 9. Relocate cardboard/foil adjacent to the fryer to provide sufficient space for oven to be located under hood. 10. Hood lights are no longer working and shall be replaced to provide sufficient lighting. 11. Burnt out light fixtures and fixtures without proper shielding at display refrigerators and hot holding unit which shall be replaced. 12. Seal prep table to adjacent wall. 13. Repair broken thermometers for the front counter deli meat display refrigerator. 14. Walk-in refrigerator light cover is broken and one fixture is not properly working and shall be repaired/replaced.
