    Observation: Observed employee handling ready to eat food, bread, with their bare hands. Employee discarded.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands.

    Observation: Observed employee handling bread with gloves leave the area return wearing same gloves and not washing their hands.

Discussed the following with the Person in Charge:
1) Agency uses kitchen facilities with other entities, however agency has dedicated cold storage equipment and dry storage areas.
2) 3 compartment sink sanitizer, chlorine @ 150ppm, cleaning bucket @ 150ppm.

    Observation: Observed single-use forks and spoons in self-service line displayed and dispensed with handles in down position.
    Correction: Handle, display, or dispense cleaned and sanitized utensils so the food- or lip-contact surface are protected from contamination.
