Inspection conducted due to a change of ownership. No food preparation was observed during today's inspection. Ensure employees are washing hands at the hand sink thoroughly and frequently while working with food and when changing tasks. Significant improvement noted in overall cleanliness and physical facility maintenance since previous routine inspections under previous owners. Continue working to correct the items mentioned above. Person in Charge was able to correctly answer food safety questions pertaining to the establishment. A verbal employee health policy is in place. A written example of an employee health policy was given to the Person in Charge. Facility is within substantial compliance with the Virginia Department of Health Food Regulations, therefore permit is issued. Ensure permit is posted in a location where it can be seen by the public.
- Food - Food Labels
Observation: Insufficient information is provided on the label of the lemonade packaged in the food establishment.
Correction: Food packaged in a food establishment for customer self service shall include labels that contain the following information: 1. the common name of the food, 2. the list of major ingredients including artificial colors or preservatives, 3. the total net weight or quantity of the food, 4. the name and place of business of the food manufacturer, packer, or distributor, 5. the name of the food source for each major food allergen contained in the food,6. nutrition labeling and, 7. for any salmonid fish containing canthaxanthin as a color additive, the labeling of the bulk fish container, including a list of ingredients, displayed on the retail container or by other written means.

Observation: The lid to the reach in freezer in the food preparation area of the facility was observed in a state of disrepair and damaged.
Correction: Repair the lid to the freezer to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability.
- Equipment - Cutting Surfaces
Observation: The cutting board along the pizza prep cooler are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: deli slicer.
Correction: Clean and sanitize these surfaces for food contact.
- Critical: Backflow Prevention Device, When Required* (corrected on site)
Observation: Observed a long hose with a nozzle attached to the mop sink faucet and the water was turned on. Backflow or backsiphonage prevention device not available on threaded faucet as required by law. Person in Charge removed nozzle and stated they will only attach nozzle while the hose is in use, or they will obtain a proper dual-check valve to install on the faucet.
Correction: Provide a backflow or backsiphonage prevention device on all threaded faucets.
- Handwashing - Using a Handwashing Lavatory (corrected on site)
Observation: The handwashing facility located at the kitchen is blocked, preventing access by employees for easy handwashing.
Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the large trash can preventing its use.
- Light Bulbs Protective Shielding
Observation: Light bulbs (2 of 3) in the exhaust hood over the cook line are not shielded, coated, or otherwise shatter-resistant.
Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.
- Outer Openings - Protected
Observation: Outer opening of the food establishment is not protected against entry of insects and rodents.Observed gap along the base of the rear door of the facility.
Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.
- Handwashing Cleanser - Availability (corrected on site)
Observation: Soap was not provided at the hand washing lavatory in the kitchen.
Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
- Physical Facilities in Good Repair
Observation: The following physical facilities are not maintained in good repair: the ceiling tile in the women's rest room is broken, and there is a section of coving missing behind the dry storage shelving unit.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
- Mops - Drying Mops
Observation: Mops not hung up to air dry.
Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
- Critical: Toxics - Storage of Toxic Containers* (corrected on site)
Observation: Containers of air freshener spray observed stored next to a box of onions on the bottom shelf of the prep table.
Correction: Store detergents, sanitizers, related cleaning or drying agents and caustics, acids, polishes and other chemicals separately from insecticides and rodenticides.
